Dulaglutide modestly raises amylase and lipase in meta-analysis of diabetes trials

A new systematic review and meta-analysis of randomized controlled trials reports that subcutaneous dulaglutide, a widely used glucagon-like peptide-1 receptor agonist, produces small but statistically significant increases in serum amylase and lipase in overweight and obese patients with type 2 diabetes mellitus. The findings, published in Diabetology & Metabolic Syndrome, address a long-standing question about whether these enzyme elevations seen with GLP-1 receptor agonists reach a magnitude that matters clinically. According to the authors, the increases remained well below thresholds considered relevant for pancreatitis, supporting the overall pancreatic safety of the drug, although monitoring may be warranted in high-risk individuals.

The researchers searched PubMed/MEDLINE, Scopus, Web of Science, and Embase up to May 2026, including randomized controlled trials that compared dulaglutide with placebo in overweight or obese adults with type 2 diabetes and reported amylase or lipase outcomes. They pooled weighted mean differences with 95% confidence intervals using a random-effects model. Seven study arms, covering 2,570 participants, contributed to the amylase analysis, while 12 arms, covering 3,041 participants, contributed to the lipase analysis.

Compared with placebo, dulaglutide significantly increased serum amylase by a weighted mean difference of 3.85 U/L (95% CI: 2.41–5.28; P < 0.001) and lipase by 6.07 U/L (95% CI: 3.87–8.26; P < 0.001). Between-study heterogeneity was substantial, with I² values of 74.1% for amylase and 81.7% for lipase, indicating considerable variation in effect sizes across the included trials. The authors note that GLP-1 receptor agonists, including dulaglutide, have previously been associated with elevations in pancreatic enzymes, but the magnitude and clinical significance of these changes in this specific population had remained unclear.

Subgroup analyses revealed that the enzyme elevations were more pronounced with shorter treatment durations of 26 weeks or less for both enzymes. Lipase increases were greater in patients with a body mass index of 30 kg/m² or higher. Sensitivity analyses confirmed the stability of the results, and the authors detected no significant publication bias. The study is a meta-analysis of trial data rather than a new clinical experiment, and its conclusions are limited to the populations and follow-up periods of the included randomized trials, which compared dulaglutide against placebo in overweight or obese adults with type 2 diabetes.
