Retrospective cohort links immune checkpoint inhibitor–containing chemoimmunotherapy to longer overall survival in recurrent nasopharyngeal carcinoma

Immune Checkpoint Inhibitors Linked to Longer Survival in Recurrent Nasopharyngeal Cancer

A retrospective cohort study of 37 patients with locoregionally recurrent nasopharyngeal carcinoma found that first-line systemic treatment containing immune checkpoint inhibitors was associated with longer overall survival than chemotherapy alone. The published abstract reports three-year overall survival of 82.7% in the chemoimmunotherapy group and 36.9% in the chemotherapy-only group, a difference reported with a P value of 0.006. The findings are observational and were described by the investigators as hypothesis-generating.

The study included patients diagnosed between July 2018 and June 2024. Of the 37 participants, 20, or 54.1%, had local recurrence, nine, or 24.3%, had regional recurrence, and eight, or 21.6%, had locoregional recurrence. Median follow-up was 46.5 months, during which 23 patients, or 62.2%, experienced disease progression. The objective response rate was 66.7% among patients who received immune checkpoint inhibitors and 61.5% among those who did not, with no statistically significant difference reported for that measure (P = 1.000).

Three-year progression-free survival was 56.0% in the chemoimmunotherapy group and 23.1% in the chemotherapy-alone group, with a reported P value of 0.012. The investigators used propensity score overlap weighting for the primary adjusted analysis and inverse probability of treatment weighting as a sensitivity analysis. In the primary overlap-weighted analysis, immune checkpoint inhibitor–containing treatment was associated with a lower risk of death, with a hazard ratio of 0.198 and a 95% confidence interval of 0.059 to 0.665 (P = 0.009). The association with progression-free survival did not reach statistical significance: the hazard ratio was 0.388, the 95% confidence interval was 0.137 to 1.101, and P = 0.075.

Among the 15 deaths recorded, 14 were attributed to tumor progression, including two cases of fatal epistaxis, representing 13.3% of all deaths. Because the evidence comes from a small retrospective cohort, it does not establish that adding immune checkpoint inhibitors caused the observed survival differences. The available abstract states that the results require further validation in prospective clinical trials. This report summarizes the published abstract rather than a full assessment of the study article.
