Systematic Review Details How Food and Alcohol Influence ADHD Drug Absorption

Food intake alters the pharmacokinetics of most medications prescribed for attention-deficit/hyperactivity disorder primarily by changing the rate rather than the extent of drug absorption, according to a systematic review synthesizing clinical evidence. The literature evaluation, which surveyed databases including PubMed, Web of Science, Scopus, Embase, and Cochrane CENTRAL, examined human clinical trials investigating ADHD drug pharmacokinetics under defined nutritional states, such as fasting versus fed conditions or alcohol co-administration. Across the 33 clinical studies meeting inclusion criteria, modified-release formulations demonstrated notable formulation-specific variability, producing alterations in peak concentration, time to peak concentration, early exposure, and concentration–time profile curves despite comparatively preserved total exposure in several trials.

The review identified clinically significant interactions regarding alcohol consumption, for which human studies were predominantly available in combination with methylphenidate. Co-administration of alcohol in these trials led to increased systemic methylphenidate exposure as well as the formation of ethylphenidate. In comparison to stimulants like methylphenidate and amphetamines, eligible published data for non-stimulant medications remained sparse, with the systematic search identifying only one eligible pharmacokinetic investigation each for extended-release clonidine and viloxazine.

Significant evidentiary limitations were documented regarding the study populations and trial designs, as most included trials were small, single-dose pharmacokinetic studies conducted in healthy adult volunteers rather than patients diagnosed with ADHD. Evidence assessing these medications under steady-state conditions remains limited. Because pharmacokinetic variability may influence therapeutic efficacy and tolerability, the review concluded that consistent administration conditions and patient counselling are important to optimize clinical outcomes.
