Retrospective Study Evaluates Relapse and Survival in Intracranial Germinoma

A retrospective cohort study of 63 patients with pure intracranial germinoma treated at Children’s Cancer Hospital Egypt between 2008 and 2024 demonstrated that whole-ventricle irradiation followed by a tumor-bed boost achieved excellent local and ventricular disease control, with recurrences predominantly characterized by delayed distant neuraxial dissemination. Over a median follow-up period of 62.9 months, recurrent disease developed in six patients, representing 9.5% of the analyzed cohort. Among the individuals who experienced disease recurrence, four patients (66.7%) subsequently died, while the estimated 7-year overall survival across the entire cohort reached 86.5% (95% CI, 78.2%-95.7%) and estimated 7-year event-free survival was 80.2% (95% CI, 70%-91.9%).

In the study protocol, patients received whole-ventricle irradiation to a dose of 23.4 Gy, followed by a tumor-bed boost up to a total dose of 45 Gy. According to the research context, intracranial germinomas are highly radiosensitive tumors associated with excellent long-term outcomes, and whole-ventricle irradiation with a tumor-bed boost—with or without chemotherapy—has become a standard treatment for localized disease designed to balance disease control with reduced treatment-related toxicity. The study objectives were specifically designed to characterize relapse patterns and describe survival outcomes under this treatment regimen.

Detailed analysis of failure patterns revealed that overall intracranial recurrence occurred in two patients (3.2%), whereas distant neuraxial dissemination represented the predominant pattern of treatment failure. Specifically, the local failure rate was 1.6%, consisting of one patient presenting with an accompanying local pineal and spinal recurrence, and the ventricular failure rate was also 1.6%, occurring in one patient. In contrast, the distant failure rate reached 7.9%, accounting for five cases of distant neuraxial dissemination involving spinal and/or cranial leptomeningeal disease. Endocrine late toxicities were identified as the main adverse sequelae observed during follow-up.

The study authors concluded that whole-ventricle irradiation followed by a tumor-bed boost provided excellent local and ventricular disease control in patients with intracranial germinoma. However, because recurrences were predominantly characterized by delayed distant neuraxial dissemination, the authors emphasized the importance of long-term surveillance despite favorable survival outcomes. The reported scope reflects findings from a single-institution retrospective cohort treated over a sixteen-year window.
