Transcriptomic study links isoform remodeling to survival differences in clear cell renal cell carcinoma

Hidden RNA Isoform Switches Reshape Kidney Cancer and Point to New Biomarkers

An analysis of 127 clear cell renal cell carcinoma (ccRCC) tumors and 33 normal-adjacent tissues found transcript-level changes involving 1,170 transcripts, two expression subgroups with different angiogenic profiles and survival differences, and preliminary prognostic candidates. The study combined formal differential transcript usage testing with a separate assessment of transcript abundance. DRIMSeq and stageR were used to evaluate shifts in the relative proportions of isoforms within genes.

Transcripts meeting the differential transcript usage criteria were then examined with a covariate-adjusted limma-voom analysis that incorporated clinical and tumor-microenvironment covariates. Cell-type proportions were also considered, allowing the tumor-versus-normal comparisons to account for differences in tissue composition. Consensus clustering based on transcript-level expression identified two ccRCC subgroups.

The abstract reports that these groups had distinct angiogenic profiles and statistically significant survival differences. Cluster-level analysis also identified differential transcript usage supported by transcript-abundance changes in genes involved in cytoskeletal organization, including ACTB; immune processes, including B2M; extracellular matrix organization, including FN1 and APLP2; and iron metabolism, including FTH1. Reported protein-domain alterations included loss of actin-associated domains in ACTB and immunoglobulin-like domains in B2M.

Cox proportional hazards modeling combined transcript-level features with clinical variables to explore prognostic associations. Twelve candidate transcripts passed an exploratory selection threshold across repeated train-test splits, providing preliminary evidence that isoform-level features may contribute prognostic information beyond clinical variables. External validation was limited to two overlapping candidates, FGFR1 and NUCB1, so the reported candidates remain preliminary rather than a clinically validated biomarker panel.

It supports the reported cohort sizes, analytical approach, transcript counts, cluster findings and exploratory prognostic results, but it does not provide the complete methods, individual transcript-level results or a full account of limitations beyond the restricted external validation. The study is therefore best understood as a human tumor transcriptomic and exploratory prognostic analysis, with its isoform-level findings tied to ccRCC pathways and outcome-associated patterns rather than presented as a validated clinical assay.
