Emergency Psychiatry Case Series Examines Chatbot Use During Youth Suicidal Crises

Seven adolescents and young adults aged 15 to 23 presenting to the psychiatric emergency department of Toulouse University Hospital in France reported interacting with ChatGPT in the context of suicidal ideation or behavior. In a retrospective observational case series analyzing medical records from routine clinical assessments conducted between January and May 2025, six of the seven identified patients were female. Diagnoses documented among the cohort included major depressive disorder in three patients, post-traumatic stress disorder in two, adjustment disorder in one, and unspecified emotional disorder in one.

Most of the reported interactions involved patients seeking information regarding the lethality of medications or other substances, accounting for six of the seven cases. Descriptively analyzed patient accounts revealed heterogeneous experiences concerning chatbot responses: five interactions were classified as potentially harmful, one was categorized as ambiguous or incomplete, and one was classified as protective. According to patients’ accounts, these interactions ranged from the reported provision of potentially harmful information to guidance that encouraged help-seeking, while the temporal relationship between the chatbot interactions and subsequent clinical events varied across cases.

Researchers emphasized that while real-world clinical observations of suicide-related generative artificial intelligence interactions remain scarce, the direct influence of chatbot communications on subsequent behaviors cannot be established from this case series. The study authors concluded that these observational findings highlight the relevance of exploring artificial intelligence interactions during routine psychiatric assessments and demonstrate the need for further research on chatbot safety among vulnerable individuals.
