A commentary published in GeroScience argues that veterinary medicine’s approach to approving lifespan-extending drugs for dogs could point the way toward developing gerotherapeutics for humans. The authors, Matthew Peloquin, Hubert C. Chen and Celine-Lea Halioua-Haubold, frame existing cardio-renal-metabolic therapies as the first wave of potential drugs aimed at extending healthy lifespan.
The commentary highlights that drugs such as GLP-1 receptor agonists and SGLT2 inhibitors, already widely prescribed for cardiometabolic conditions, have accumulated evidence of all-cause mortality benefits. The authors position these therapies as candidates for repurposing toward a lifespan extension indication, rather than requiring entirely new molecules to be discovered.
However, the authors identify substantial barriers to repurposing approved drugs for longevity. Clinical trials of sufficient duration to demonstrate lifespan effects are costly and slow, commercial incentives are misaligned with such an indication, and no viable regulatory pathway currently exists to approve a drug specifically for extending human lifespan.
To address these obstacles, the commentary proposes two parallel strategies. The first is identifying and validating precise surrogate endpoints, which could serve as stand-ins for mortality outcomes in shorter trials. The second is modifying the FDA’s 505(b)(2) regulatory pathway to allow accelerated approval of already-approved drugs that have demonstrated mortality benefit and favorable post-marketing safety.
As a precedent, the authors point to veterinary medicine’s expanded conditional approval framework, which already applies both strategies for canine lifespan therapeutics. They argue that this regulatory model offers a transferable template for human gerotherapeutics development, bridging lessons from companion animal medicine to human aging research.
