Bariatric Surgery Tied to Improved Fertility-Sparing Cancer Therapy in Retrospective Study

In a retrospective cohort study evaluating adjunctive interventions for uterine conditions, bariatric surgery was identified as an independent predictor of achieving complete remission among morbidly obese women undergoing fertility-sparing treatment for endometrial cancer or atypical endometrial hyperplasia. After researchers adjusted for pathology, patients who underwent bariatric surgery exhibited a significantly higher likelihood of complete remission compared to those in the non-surgery group, with an adjusted hazard ratio of 7.82 (95% CI: 1.81–33.78; p=0.006). Survival analysis additionally revealed a tendency toward an improved cumulative complete remission rate in the surgical cohort relative to the non-surgical cohort (p=0.053).

The retrospective investigation evaluated 25 morbidly obese women with endometrial cancer or atypical endometrial hyperplasia who received fertility-sparing treatment between 2022 and 2024 to address the less than ideal effectiveness of fertility-preserving approaches in this population. Alongside oncologic remission trends, patients who received bariatric surgery demonstrated marked improvements in metabolic parameters, including reductions in body mass index, glycated hemoglobin, and fasting insulin levels. These metabolic shifts were accompanied by a decrease in systemic inflammation markers, specifically lower interleukin-6 and neutrophil counts, as well as enhanced antitumor immunity characterized by increased circulating CD8⁺ T cells and natural killer cells.

Laboratory evaluations further demonstrated that the tumor proliferation marker Ki-67 was significantly decreased in patients undergoing bariatric surgery compared to the non-surgical group (p=0.03). Network correlation analysis linked obesity-related metabolic dysfunction directly to pro-inflammatory and tumor-proliferative biological pathways. Based on these observed associations, the authors concluded that bariatric surgery may serve as a superior weight-loss intervention for morbidly obese endometrial cancer patients receiving fertility-sparing therapy, likely exerting its beneficial effects through the modulation of metabolic, inflammatory, and immune pathways.
