Short Sleep Linked to Higher LDL Cholesterol in Children with Obesity

Children with obesity who sleep fewer hours may have higher LDL cholesterol levels, according to a new cross-sectional analysis published in Eating and Weight Disorders – Studies on Anorexia, Bulimia and Obesity.

The analysis drew on baseline data from a randomized clinical trial designed to treat pediatric obesity, meaning the measurements were taken before any treatment began. Sleep duration was self-reported, and fasting blood samples were collected after 12 hours to measure total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. On average, the children slept 8.61 hours per day, with a standard deviation of 1.58 hours, and 48.5 percent reported sleeping less than 9 hours per day.

The researchers used generalized linear models adjusted for sex, age, skin color, and physical activity to test whether sleep duration was associated with each lipid marker. Crucially, they stratified the sample by obesity severity, defining non-severe obesity as a BMI z-score between +2 and +3 and severe obesity as a BMI z-score above +3. This distinction proved decisive for the main finding.

In children with non-severe obesity, each additional hour of sleep was associated with an LDL cholesterol concentration 4.11 mg/dL lower, a statistically significant inverse association with a p-value of 0.04. No comparable association emerged in the severe obesity group, where average LDL cholesterol was 84.2 mg/dL compared with 92.1 mg/dL in the non-severe group. The overall mean LDL cholesterol across the sample was 88.4 mg/dL with a standard deviation of 25.5 mg/dL.

Sleep and obesity severity were themselves intertwined. Children with short sleep were more likely to have severe obesity, with 57.1 percent of short sleepers falling into that category, a difference that reached the threshold of statistical significance at a p-value of 0.05.

The study carries the designation of Level III evidence, described as a well-designed cross-sectional analytical cohort study, and its conclusions are correspondingly cautious. Because the data are cross-sectional, sleep duration and cholesterol were measured at a single point in time, so the findings show an association rather than a causal effect. Self-reported sleep is also vulnerable to misreporting, and the modest sample size of 101 children limits the precision of the estimates.

The present study adds a nuance: the strength of that relationship may depend on how severe a child’s obesity is.

The practical implication, the authors write, is that promoting healthy sleep habits could be a useful component of comprehensive obesity management in pediatric populations.
