Research report: Subdivision of metachronous metastatic nasopharyngeal carcinoma based on the framework of TNM-9: A recursive partitioning analysis of real-world data

A real-world analysis of 218 patients with metachronous metastatic nasopharyngeal carcinoma found that adding metastatic-lesion count and disease-free interval to the version nine AJCC/UICC TNM M1 category produced more distinct prognostic groups. Patients classified as M1a under TNM-9 had a three-year overall survival rate of 63.2%, compared with 40.8% for those classified as M1b, according to the study. The investigators then used recursive partitioning analysis to derive a proposed RPA-M1 classification, which separated patients into groups with three-year overall survival rates of 74.6% and 40.6%.

The study included patients with newly diagnosed metachronous metastatic nasopharyngeal carcinoma treated between 2015 and 2024. Of the 218 participants, 111, or 50.9%, were classified as M1a and 107, or 49.1%, as M1b. Multivariable Cox analysis identified both the TNM-9 M1 category and disease-free interval as independent prognostic factors for overall survival, with both P values below 0.05.

The analysis therefore supported the prognostic relevance of the TNM-9 M1 category while showing that the timing of metastatic disease and the number of metastatic lesions supplied additional prognostic information in this population. Recursive partitioning analysis generated two groups based on these variables. RPA-M1a consisted of patients with no more than three metastatic lesions and a disease-free interval longer than 12 months.

RPA-M1b included patients with more than three metastatic lesions or a disease-free interval of 12 months or less. The lower-risk RPA-M1a group had a three-year overall survival rate of 74.6%, whereas the RPA-M1b group had a rate of 40.6%, a difference evaluated at P < 0.001. These results suggest that combining metastatic burden with disease-free interval distinguished prognosis more clearly than the TNM-9 M1 category alone.

Model performance was evaluated with Harrell’s C-index, time-dependent areas under the receiver operating characteristic curve, the Akaike information criterion, calibration and decision curve analysis. The researchers described the prognostic performance of the RPA-M1 classification as modestly but consistently improved. Treatment analyses also suggested differences by risk group: metastasis-directed therapy was associated with longer overall survival only in RPA-M1a, while an association with benefit from intensified systemic therapy was reported primarily in RPA-M1b.

The authors characterized the findings as hypothesis-generating and called for independent external validation.
