A single-center retrospective study has reported that orelabrutinib, a highly selective Bruton’s tyrosine kinase (BTK) inhibitor, produced encouraging results when given as first-line monotherapy to patients with treatment-naive chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL). The findings, published as an open-access article in the Journal of Cancer Research and Clinical Oncology, add real-world evidence for a drug class that has become a standard of care in this disease.
The study included 75 patients with CLL/SLL who received orelabrutinib monotherapy as their first treatment between November 27, 2021, and November 20, 2024. Because it was a retrospective analysis of routine clinical practice at a single center, the results reflect how the drug performed outside the controlled setting of a randomized trial, though they also carry the limitations inherent to that design, including the absence of a comparison group.
The primary outcome was the objective response rate (ORR). Among the 75 patients, the ORR and the disease control rate were both 86.7%, with a 95% confidence interval of 76.8 to 93.4. In other words, roughly nine in ten patients saw their disease respond to or come under control with the single-agent BTK inhibitor.
With a median follow-up of 30.8 months, ranging from 2.1 to 48.1 months, the median progression-free survival had not been reached at the time of analysis. The 12-month progression-free survival rate was 85.1% (95% CI, 77.4–93.6), and the 24-month rate was 78.0% (95% CI, 69.0–88.2). Overall survival was similarly favorable: the median overall survival was not reached, and both the 12- and 24-month overall survival rates stood at 93.3% (95% CI, 87.9–99.2).
Safety data were a central part of the report. Treatment-related adverse events occurred in 68.0% of patients, a figure the authors describe as consistent with an acceptable safety profile. Importantly, grade 3 or higher treatment-related adverse events were uncommon, occurring in just 3 of 75 patients (4.0%). These comprised anemia in one patient (1.3%), decreased neutrophil count in one patient (1.3%), and lung infection in one patient (1.3%).
No treatment-related deaths were reported in the cohort.
Orelabrutinib is described in the study as a novel, potent BTK inhibitor with high selectivity. BTK inhibitor-based regimens have become standard treatment for CLL/SLL as targeted therapy has advanced, but the authors note that real-world evidence for orelabrutinib specifically in treatment-naive patients has remained scarce, which motivated the present analysis.
The authors conclude that orelabrutinib demonstrated promising clinical outcomes and an acceptable safety profile in treatment-naive patients with CLL/SLL in routine clinical practice. They frame the findings as supportive real-world evidence that warrants confirmation in larger multicenter cohorts, underscoring that these results are hypothesis-generating rather than definitive.
Larger, multi-institutional studies will be needed to establish whether these single-center results hold more broadly.
