Nanocarriers May Boost Berberine's Anticancer Activity in Triple-Negative Breast Cancer

A narrative review published in Discover Oncology examines how nanotechnology-based delivery systems could improve the effectiveness of berberine, a plant-derived compound with reported anticancer activity, against triple-negative breast cancer (TNBC). The authors, Javed Ahamad and Gudisa Bereda, note that TNBC is the most aggressive breast cancer subtype and remains difficult to treat because of its heterogeneity, metastatic potential, and resistance to therapy. Berberine shows promising anticancer activity in preclinical work, but its clinical translation is constrained by pharmacokinetic and delivery problems, including poor solubility, low bioavailability, rapid metabolism, and inadequate tumor targeting.

The review, which followed the SANRA framework and drew on searches of PubMed, Scopus, Web of Science, and Google Scholar, synthesized findings from preclinical studies. According to the authors, berberine appears to suppress TNBC progression by modulating multiple signaling pathways, including PI3K/AKT/mTOR, EGFR/MEK/ERK, NF-κB, STAT3, AMPK, TGF-β, Wnt/β-catenin, and NLRP3.

The core of the review concerns nanoformulations designed to overcome berberine’s delivery limitations. The surveyed platforms include polymeric nanoparticles, lipid-based nanocarriers, dendrimers, micelles, inorganic nanoparticles, hybrid systems, and stimuli-responsive carriers. The authors report that these systems improved berberine’s solubility, bioavailability, stability, tumor accumulation, and intracellular delivery, and that these improvements enhanced therapeutic efficacy in the preclinical studies they appraised. Among the platforms, polymeric and lipid-based nanocarriers were judged to have the greatest translational potential because of their favorable efficacy, biocompatibility, and scalability.

The review is equally explicit about what stands between these laboratory results and patient use. Clinical translation remains limited by insufficient human evidence, safety concerns, manufacturing complexity, regulatory uncertainty, and the lack of standardized formulations. Because the evidence base is qualitative and preclinical, the findings should be read as a map of promising research directions rather than as demonstrated clinical benefit; no nano-berberine formulation has yet been validated in well-designed clinical trials for TNBC.

The authors conclude that nano-berberine represents a promising strategy for TNBC treatment, but that realizing it will require standardized formulations, rigorous safety assessments, scalable production, regulatory alignment, and well-designed clinical trials. Until such trials are conducted, the review’s contribution lies in consolidating the mechanistic and formulation evidence, and in identifying the specific translational barriers—rather than in establishing a new therapy.
