Zipalertinib plus chemotherapy extends survival in EGFR exon 20 lung cancer

Adding the oral EGFR inhibitor zipalertinib to platinum-pemetrexed chemotherapy significantly extended progression-free survival in patients with previously untreated advanced non-small cell lung cancer harboring EGFR exon 20 insertion mutations, according to phase 3 results presented at the International Association for the Study of Lung Cancer 2026 World Conference on Lung Cancer in Seoul. In the randomized REZILIENT 3 trial, median progression-free survival assessed by blinded independent central review was 14.5 months with the combination compared with 8.5 months with chemotherapy alone, corresponding to a 50% reduction in the risk of disease progression or death (hazard ratio 0.50; 95% confidence interval, 0.34–0.73; P=0.00015). Investigators reported that the benefit was consistent across prespecified subgroups, including patients with brain metastases, who showed a hazard ratio of 0.38.

The trial enrolled 279 patients with advanced NSCLC and EGFR exon 20 insertion mutations who had received no prior treatment for advanced disease. Participants were randomly assigned 1:1 to platinum-pemetrexed chemotherapy plus zipalertinib 100 mg twice daily (n=140) or to chemotherapy alone (n=139). Patients with untreated, asymptomatic brain metastases up to 2 cm were eligible for enrollment.

Patients assigned to chemotherapy alone were permitted to cross over to zipalertinib after disease progression. Beyond delaying progression, the combination produced substantially higher tumor response rates. The objective response rate was 65.0% with zipalertinib plus chemotherapy versus 40.3% with chemotherapy alone, a difference that was statistically significant (P<0.0001).

Responses also lasted longer: median duration of response was 14.2 months in the combination arm compared with 9.9 months in the chemotherapy-only arm. “REZILIENT 3 demonstrated that adding zipalertinib to platinum-based chemotherapy produced a statistically significant and clinically meaningful six-month improvement in progression-free survival for patients with advanced NSCLC and EGFR exon 20 insertion mutations,” said Professor Daniel Tan of the National Cancer Centre Singapore and Duke-NUS Medical School. “The combination also produced a substantially higher response rate, supporting its potential as a first-line treatment approach for this patient population.”

The interim overall survival analysis, which had reached 30% maturity at the time of reporting, showed a hazard ratio for death of 0.72 (95% confidence interval, 0.42–1.23) favoring the combination. Because this interim analysis was based on only 30% of the expected survival events, the confidence interval crosses 1, and investigators emphasized that follow-up is ongoing to further characterize overall survival and exploratory endpoints. The trial is registered as NCT05973773.

The safety profile of the combination was described as generally consistent with the known safety profiles of the individual agents. Grade 3 or higher adverse events occurred in 87.1% of patients receiving zipalertinib plus chemotherapy and 54.4% of those receiving chemotherapy alone, an increase driven primarily by hematologic events that investigators characterized as manageable. Grade 3 or higher EGFR-related toxicities were infrequent and occurred only in the combination arm, comprising rash in 10.7% of patients and diarrhea in 1.4%.

No new safety signals were observed in the trial. The investigators concluded that adding zipalertinib to platinum-based chemotherapy provided a statistically significant and clinically meaningful improvement in progression-free survival as first-line treatment for advanced NSCLC with EGFR exon 20 insertion mutations. The findings support the combination as a potential first-line approach for this molecularly defined population, though the reported endpoints reflect the trial's interim stage: progression-free survival and response outcomes were statistically significant, while the overall survival result remains immature and inconclusive.

The results were presented at the IASLC 2026 World Conference on Lung Cancer, the world's largest meeting dedicated to lung cancer and other thoracic malignancies, which brings together researchers, physicians, and specialists from more than 100 countries.
