Social Media Exposure Linked to Teen Substance Use in India, Parents Moderate Risk

Digital exposure is emerging as a measurable risk factor for adolescent substance use in India, and a new multicentre study suggests that conventional parental oversight may do far less to blunt that risk than families and policymakers might hope. The research, published in the International Journal of Mental Health and Addiction, drew on baseline data from more than 6,100 school-going adolescents aged 10 to 19 years across six geographically diverse regions of the country and found a striking dose–response relationship between the intensity of a young person’s digital life and the likelihood of recent substance use.

The investigation was designed to address a persistent gap in the literature. Most of what is known about the links between screen-based behaviour and health-risk behaviours such as smoking, drinking, and drug use comes from high-income countries, where device penetration, platform ecosystems, and family structures differ markedly from those in low- and middle-income settings. India, with one of the world’s largest adolescent populations and one of its fastest-expanding digital user bases, offers a critically important context in which to test whether the patterns observed elsewhere hold true. The study team, led by U. Venkatesh of the Department of Community and Family Medicine at the All India Institute of Medical Sciences, Gorakhpur, along with collaborators at AIIMS institutions in Guwahati, Nagpur, Deoghar, and Rajkot, at JIPMER Puducherry, and in the department of psychiatry at AIIMS Gorakhpur, set out to quantify both the association between multidimensional digital exposure and current substance use, and the extent to which parental monitoring modifies that association.

To capture the school-based adolescent population fairly, the researchers used a multistage cluster sampling method, ultimately analysing data from 6,168 participants. The outcome of interest was current substance use, defined as use within the preceding three months, and it was assessed with the World Health Organization’s Alcohol, Smoking and Substance Involvement Screening Test, widely known as WHO-ASSIST. This validated instrument, whose psychometric properties were established in an international validation study published in the journal Addiction, allows researchers to classify substance involvement across multiple categories rather than relying on a single yes-or-no question, lending the prevalence estimates considerably more credibility than many school surveys achieve.

Perhaps the study’s most important methodological contribution lies in how it operationalised digital exposure. Rather than treating “screen time” as a single number of hours, the team constructed a composite index that integrated four distinct dimensions: the intensity of social media use, direct exposure to substance-related content online, late-night device use, and perceived peer pressure originating from digital platforms. Composite indicators of this kind are well established in the measurement literature — the researchers cite the OECD handbook on constructing composite indicators and subsequent work on weighting and aggregation — because they capture a phenomenon that is genuinely multidimensional. A teenager who scrolls social media for hours, encounters alcohol or tobacco imagery, stays awake past midnight with a phone in hand, and feels social pressure to conform to behaviours celebrated in their feeds is experiencing something qualitatively different from a teenager who merely spends long hours watching video content. By bundling these dimensions into a single index and then stratifying participants into low, moderate, and high exposure categories, the researchers could test whether risk rises in step with the overall intensity of digital immersion.

The results were unambiguous. Current substance use was reported by 11.2 percent of the adolescents overall. But that headline figure conceals a steep gradient: prevalence climbed from 6.7 percent in the low-exposure group to 16.6 percent in the high-exposure group, a difference that was highly statistically significant with a p-value below 0.001. In other words, adolescents in the highest digital exposure tier were roughly two and a half times more likely to report recent substance use than their least-exposed peers. That kind of monotonic, dose–response pattern is precisely what epidemiologists look for when they want to distinguish a plausible exposure–outcome relationship from statistical noise, and it echoes findings from a 2023 systematic review and meta-analysis in the BMJ by Purba and colleagues, which linked social media use to a range of health-risk behaviours in young people.

When the team fitted multivariable logistic regression models to adjust for potential confounders, the gradient persisted. Compared with low exposure, moderate digital exposure was associated with an adjusted odds ratio of 1.63 for current substance use, with a 95 percent confidence interval of 1.19 to 2.23, meaning that even after accounting for other measured differences between participants, moderately exposed adolescents had about 63 percent higher odds of recent substance use. In the high-exposure category, the adjusted odds ratio rose to 2.20, with a 95 percent confidence interval of 1.55 to 3.11. Because the confidence intervals exclude 1.0 in both cases, the associations are statistically robust at conventional thresholds, and the increasing magnitude across categories reinforces the dose–response interpretation. The plausible biological and social mechanisms behind such a relationship are not hard to imagine: algorithms that surface substance-related imagery to curious young users, the normalising effect of peer displays of drinking or smoking in curated feeds, sleep disruption from late-night device use that erodes self-regulation, and the amplification of peer pressure through always-on connectivity — mechanisms that prior work, including a pilot ecological momentary assessment study published in Frontiers in Child and Adolescent Psychiatry, has begun to document in real time.

The more provocative finding, however, concerns what the researchers call effect modification by parental monitoring. Theoretically, parental monitoring — the degree to which parents know where their children are, what they are doing, and who they are with — has long been considered one of the strongest protective factors against adolescent risk behaviour, a proposition rooted in classic work such as DiClemente and colleagues’ 2001 study in Pediatrics. If digital platforms are now a primary venue in which risk exposure occurs, one might expect vigilant parents to buffer much of that risk. The Indian data tell a more sobering story. Interaction analyses, formally testing whether the digital exposure–substance use association differed across levels of parental monitoring, found that monitoring provided only modest and inconsistent protection. Crucially, among adolescents with high digital exposure, the elevated odds of substance use persisted regardless of how closely parents reported monitoring their children. The digital environment, in other words, appears to partially escape the protective reach of the household.

This result carries real conceptual weight. It suggests that the familiar framing of parental supervision as a sufficient safeguard may need revision in an era when exposure to risky content, social comparison, and peer influence happens through a device that often sits in the adolescent’s pocket around the clock. It also aligns with a growing body of scholarship on digital parenting — including Modecki and colleagues’ 2022 systematic review in Perspectives on Psychological Science — which argues that measurement of parental involvement online has lagged far behind the realities of adolescent media life. If monitoring, as traditionally measured, does not meaningfully attenuate the association between digital exposure and substance use, then interventions may need to shift from surveillance-oriented approaches toward strategies that address the content and social dynamics adolescents encounter online, including media literacy, algorithmic transparency, and platform-level restrictions on substance-related marketing to minors.

The authors are careful to note the limitations inherent in their design. The study is cross-sectional, capturing a single moment in time, which means it cannot establish whether digital exposure precedes substance use, follows it, or reflects some shared underlying propensity — perhaps sensation-seeking adolescents both seek richer digital stimulation and are more open to experimentation. Longitudinal research, the team writes, is needed to clarify these temporal relationships and to inform prevention strategies. Self-reported measures of both digital behaviour and substance use are also vulnerable to recall and social desirability bias, particularly among younger respondents, although the use of the validated WHO-ASSIST tool and the guarantee of confidentiality mitigate some of these concerns. The composite index, while methodologically defensible, necessarily involves choices about weighting and categorisation that could influence where individuals fall within exposure strata.

Nevertheless, the scale and geographic breadth of the study — spanning institutions from Puducherry in the south to Guwahati in the northeast, and from Gorakhpur in the north to Rajkot in the west — give its findings a weight that single-site studies rarely achieve. The research was supported by the Indian Council of Medical Research under an extramural research grant and forms part of a broader programme evaluating community-based, peer-led interventions to prevent substance use among adolescents within the school health programme under Ayushman Bharat, India’s flagship health initiative. Baseline findings of this kind are exactly what such intervention trials need: they identify the risk landscape that any prevention effort must navigate.

For parents, educators, and public health officials, the message is double-edged. On one hand, the study confirms that heavy digital immersion — particularly exposure to substance-related content and digitally mediated peer pressure — is associated with substantially elevated odds of adolescent substance use in a large, diverse Indian sample. On the other, it cautions that simply telling parents to watch their children more closely is unlikely to neutralise that risk. The digital world is not merely another setting that supervision can police; it is a continuously evolving social environment with its own normative pressures. As adolescents in India and across the Global South come online in ever greater numbers, the study suggests that protecting them will require rethinking where, and how, prevention happens — moving beyond the living room and into the architectures of the platforms themselves.
