Shear wave elastography tracks earlier exercise responses in type 2 diabetes

Shear wave elastography detected a change in the right gastrocnemius muscle after one week of high-intensity interval exercise in a 12-week study of people with type 2 diabetes, according to the exploratory methodological trial. The investigators said the findings support further investigation of elastography as an early monitoring tool, but functional validation remains necessary. Sixty-six participants completed the 12-week intervention and were included in the per-protocol analysis: 32 in the high-intensity interval exercise group and 34 in the moderate-intensity aerobic exercise group.

Measurements were taken at baseline, week 1, week 8 and week 12. The investigators recorded HbA1c, the thickness of the medial head of the right gastrocnemius and shear wave velocity. They analyzed temporal patterns with independent t-tests, paired t-tests and repeated-measures analysis of variance.

This design allowed them to examine changes within each exercise group and differences in how outcomes varied between groups and measurement times. Shear wave velocity increased significantly by week 1 in the high-intensity interval exercise group, with p less than 0.05. These changes preceded the significant improvements in HbA1c and gastrocnemius muscle thickness, which occurred at weeks 8 and 12, respectively.

The analysis also found significant group-by-time interactions for all three outcomes; every interaction had a p value below 0.05 and a partial eta-squared above 0.06, indicating differences in the observed outcome patterns across the two exercise programs over time. The study suggests that shear wave elastography may register a measurable muscle response sooner than the other outcomes evaluated, but the evidence does not establish that the technique predicts long-term clinical benefits or replaces other measures used in diabetes management. Its scope was limited to one muscle and one side of the body, and the per-protocol analysis excluded six of the 72 enrolled participants.

Described by the researchers as exploratory and methodological, the trial indicates an earlier signal under the tested conditions rather than a clinically validated method for determining whether an exercise intervention is effective; the authors specifically called for additional functional validation.
