For women living with metastatic breast cancer, a smartphone app may offer more than convenience: it can function as a weekly clinical safety net. A nationwide randomized clinical trial led by researchers at Charité – Universitätsmedizin Berlin and the German Cancer Society found that regularly collecting patients’ symptom reports and responding to clinically important changes reduced fatigue, improved physical functioning, and lowered the need for hospitalization. The findings, published in JAMA Oncology, support the integration of the PRO B digital treatment model into routine cancer care in Germany.
Metastatic breast cancer occurs when malignant cells spread from the breast to distant organs. Although the disease is generally considered incurable, modern systemic treatments—including targeted therapies, endocrine therapies, chemotherapy, and antibody-based medicines—can control tumor growth and extend survival for years. Many of these treatments can be taken at home, reducing the frequency of hospital visits. That shift, however, can make it more difficult for medical teams to detect worsening symptoms, treatment toxicity, or a decline in patients’ ability to manage everyday activities.
The PRO B approach was designed to close that monitoring gap. At its center is a digital application that asks patients up to 52 structured questions once a week. The questions address physical symptoms, treatment-related problems, daily functioning, and emotional well-being. Patients may be asked whether they experienced pain, felt unusually exhausted, became short of breath, or were too tired to perform their usual activities. Rather than waiting for the next scheduled appointment, the system creates a continuous stream of patient-reported outcome data.
The responses are assessed by an algorithm programmed to identify clinically relevant changes. When a patient’s answers indicate a potential problem, the treatment team receives an alert and contacts her within 48 hours. The resulting intervention may be as simple as providing guidance on symptom management or adjusting supportive medication, such as an anti-nausea drug. In other cases, the alert can prompt an urgent hospital assessment. This alert-based design is important because it converts subjective experiences—such as increasing fatigue—into information that can trigger timely clinical action.
The trial included 909 patients treated at 52 breast cancer centers across Germany. Participants ranged in age from 19 to 81 and were followed for one year while receiving drug-based therapy for metastatic disease. Approximately half were assigned to receive standard treatment alongside the PRO B intervention. The comparison group was asked about well-being only once every three months and did not receive the same systematic, alert-triggered contact from care teams. The study therefore tested not merely whether patients could use an app, but whether frequent digital surveillance linked to human clinical responses could alter outcomes.
Patients in the PRO B group reported significantly less cancer-related fatigue than those in the comparison group. Cancer-related fatigue is not equivalent to ordinary tiredness: it can be physical, cognitive, or emotional, and may persist despite rest. It can interfere with employment, mobility, self-care, and social life. Participants using the digital monitoring system also rated their physical functioning more positively and experienced fewer hospitalizations. These results suggest that early recognition and management of symptoms may prevent relatively manageable problems from escalating into complications requiring inpatient care.
The researchers also report indications that the approach may extend remaining lifespan for some patients, although the most clearly emphasized benefits were improvements in fatigue, physical functioning, and hospitalization rates. The underlying mechanism is not a new anticancer drug, but a change in the timing and organization of care. By collecting data at regular intervals, clinicians can detect changes between appointments, distinguish expected side effects from warning signs, and intervene before a patient’s condition deteriorates substantially. The model also gives patients access to medically reviewed information and a direct route to professional advice, including guidance about fever, medication use, and whether emergency care may be necessary.
For participants, the psychological effect of this connection was as important as the medical response. Stephanie Neumann-Johnston, a 51-year-old participant, said the app helped her interpret symptoms and decide when to seek care, particularly outside normal hospital hours. Her experience illustrates how patient-reported outcome monitoring can redistribute some control to people living with an unpredictable illness. Instead of carrying uncertainty alone until the next appointment, patients can document symptoms as they occur and receive an assessment from the team responsible for their treatment.
The findings have already influenced health policy. Following a medical and health-economic assessment, Germany’s Federal Joint Committee recommended incorporating the PRO B treatment concept into standard care for patients with metastatic breast cancer covered by statutory health insurance. The participating insurers—BARMER, DAK-Gesundheit, and mkk – meine krankenkasse—are preparing further developments intended to make the service available before full implementation. The model will continue under the name “ida.” In a post-trial survey, more than 90 percent of patients in the PRO B group said they wanted the approach to become part of routine care, underscoring the demand for digital systems that combine automated monitoring with rapid, personal clinical support.
