Personality Pathways Shape Self-Injury Patterns in Suicidal Adolescents

Adolescents admitted to a pediatric hospital after suicidal ideation or attempts show distinct patterns of non-suicidal self-injury, and a new study suggests that different forms of personality pathology point to different clinical dimensions of that self-harm. The research, published in Borderline Personality Disorder and Emotion Dysregulation, examined associations between NSSI outcomes and personality pathology, mood disorders, early life adversity, and suicidal ideation.

The researchers examined 118 adolescents admitted to a pediatric hospital for suicidal ideation or attempts. Standardized interviews (the C-SSRS, K-SADS, and SCID-II) were administered to assess suicidal ideation, NSSI characteristics, personality pathology, and mood disorders. NSSI was evaluated in terms of presence, duration, frequency, and severity over a 9-month observation window.

Correlational, linear, and logistic regression analyses were conducted to examine associations between the NSSI outcomes and personality pathology, mood disorders, early life adversity, and suicidal ideation.

The results point to a differentiated picture. Borderline personality features emerged as significant predictors of NSSI severity and duration. In other words, adolescents with stronger borderline features tended to show more severe self-injury that had persisted longer, within this clinical sample.

Broader non-borderline personality pathology showed a different signature: it specifically predicted repetitive, high-frequency self-harm patterns. This separation matters clinically, because severity, duration, and frequency are distinct dimensions that may call for different assessment and intervention priorities.

Suicidal ideation was associated with prolonged duration of NSSI, but it did not independently predict high-frequency self-harm. Mood disorders showed limited independent predictive value once the other factors were considered. The authors suggest these findings indicate differentiated pathways linking personality pathology and suicidal ideation to distinct clinical dimensions of adolescent NSSI.

The study’s framing reflects a long-standing clinical debate. Non-suicidal self-injury is conceptually distinct from suicidal behavior, yet the two frequently co-occur and may share underlying psychopathological pathways. Borderline personality disorder has long been considered a key factor in the emergence and persistence of NSSI, but recent evidence suggests that other dimensions of personality pathology, together with mood disorders and suicidal ideation, may significantly influence the onset, severity, and course of self-injurious behaviors in adolescence.

Based on these findings, the authors conclude that integrating the assessment of personality dysfunction with the evaluation of suicidal ideation may enhance clinical management and prevention strategies for adolescents at risk.
