Methylated ultra-short cfDNA fragments flag breast cancer with perfect test accuracy

Cell-free DNA is a promising source of biomarkers for early cancer detection. The study notes that cfDNA fragmentation patterns and methylation profiles exhibit coordinated changes, and the researchers set out to examine both kinds of information in the same circulating DNA. Although previous studies have explored cfDNA methylation or fragmentomics individually for breast cancer detection, a systematic evaluation of integrated methylated fragmentomic features has remained limited. A new study in BMC Cancer set out to identify cfDNA methylation markers and methylated fragmentation markers for accurate breast cancer detection.

The research team enrolled 25 breast cancer patients and 110 healthy controls and performed comprehensive profiling of methylation and methylated fragmentation patterns of cfDNA using the cfMeDIP-seq method. The researchers used cfMeDIP-seq to perform comprehensive profiling of methylation and methylated fragmentation patterns of cfDNA, focusing on fragments that carry methylation marks.

The analysis revealed a striking difference between methylated and non-methylated fragment populations. Methylated fragment features were significantly different from non-methylated fragment features, and one finding stood out: the proportion of ultra-short fragments in breast cancer patients was lower than that in non-cancer controls. In this study, the proportion of ultra-short fragments in breast cancer patients was lower than that in non-cancer controls, a difference observed in the methylated fragment features.

From this data, the researchers extracted seven types of features spanning methylation and methylated fragments. The researchers developed a stacked machine-learning model using the extracted features.

The performance reported was remarkable. The stacked model achieved an AUC of 1 in both the test and validation sets, indicating high sensitivity and specificity. The authors report that an AUC of 1 in both the test and validation sets indicates high sensitivity and specificity.

The conclusion drawn by the team is that cfDNA methylation and methylated ultra-short fragmentomics biomarkers derived from the cfMeDIP-seq method can effectively diagnose breast cancer, indicating potential clinical utility.

Several caveats deserve attention. The study included 25 breast cancer patients and 110 healthy controls, so the model has not been tested against patients with benign breast disease or other cancers. The reported performance comes from the test and validation sets of this study, and the authors' conclusion that these biomarkers can effectively diagnose breast cancer indicates potential clinical utility. This version is shared early to provide faster access to peer-reviewed, accepted research; it is citable and carries a permanent DOI, and is subject to further edits before the final Version of Record.

The study demonstrates that cfDNA methylation and methylated ultra-short fragmentomics biomarkers derived from the cfMeDIP-seq method can effectively diagnose breast cancer in this cohort. By identifying cfDNA methylation markers and methylated fragmentation markers, including the lower proportion of ultra-short fragments in breast cancer patients, and combining them in a stacked model, the study evaluated integrated methylated fragmentomic features for breast cancer detection. The authors conclude that these biomarkers indicate potential clinical utility for breast cancer detection.

Subject of Research: Detection of breast cancer using cfDNA methylation and methylated ultra-short fragment features profiled by cfMeDIP-seq.
