Household cluster-randomized trial finds fewer symptom-defined respiratory episodes with study hand sanitizer

Long-Lasting Hand Sanitizer Formulation Cuts Respiratory Illness Risk by 40 Percent in Household Trial

A double-blind, parallel-group, household cluster-randomized controlled trial reported that repeated use of a study hand-sanitizer formulation was associated with fewer upper respiratory symptoms and illness-related work restrictions than a commercially available 79.7% ethanol-based sanitizer. The findings are summarized in the published abstract; the outcome represented a defined symptom complex rather than laboratory-confirmed infection.

The trial enrolled 568 healthy employees and their cohabiting family members across 417 household clusters. Participants were assigned to use either the study formulation or the ethanol-based comparator five to 10 times daily for 72 days, from December 2023 through February 2024. The primary endpoint was cumulative incidence of upper respiratory symptoms, defined as fever of at least 37.8 °C accompanied by cough, sore throat and/or nasal discharge.

Upper respiratory symptoms occurred in 35 participants in the study-formulation group and 58 in the ethanol-based sanitizer group. The reported hazard ratio was 0.58, with a 95% confidence interval of 0.37–0.89 and P = 0.013. The abstract characterized this as approximately 40% lower cumulative incidence in the study-formulation group. Illness-related work restrictions were also fewer, although the abstract did not provide the number of restrictions or effect estimate for that outcome.

Adverse events were infrequent and were reported as unrelated to either intervention. The available evidence supports the conclusion that the study formulation was well tolerated and was associated with fewer defined upper respiratory symptoms and work restrictions during the 72-day trial period. Because the supplied material consists only of the abstract and accepted early-access version, details about the formulation’s composition, blinding procedures, setting, analysis methods and study limitations cannot be assessed here, and the symptom-based results should not be interpreted as evidence of reduced confirmed respiratory infection.
