Generic Piracetam Tablet Proves Bioequivalent to Nootropyl in Healthy Chinese Volunteers

A single 800-milligram tablet of a newly developed generic piracetam formulation has been shown to deliver the same drug exposure as the original branded product Nootropyl in healthy Chinese volunteers, whether taken on an empty stomach or after a high-fat meal. The finding comes from a randomized, open-label, two-period crossover bioequivalence study conducted at the Phase I Clinical Research Center of Wuhan Pulmonary Hospital and published in the journal Drugs in R&D. Because the two formulations produced overlapping concentration–time profiles and confidence intervals well inside the regulatory acceptance window, the authors conclude that the generic tablet can be considered clinically interchangeable with the reference product in terms of both the rate and the extent of absorption.

Piracetam is a cyclic derivative of the neurotransmitter precursor γ-aminobutyric acid, first synthesized by Giurgea at UCB Laboratories in Belgium and in clinical use since 1972 as the founding member of the so-called nootropic class. Over the decades it has been prescribed for age-related mental impairment, cognitive deterioration after cerebral ischemia, chronic cerebrovascular disorders, myoclonic epilepsy, sickle cell anemia, posture and gait disturbances, and breath-holding spells, among other indications. Although its precise mechanism of action remains incompletely understood, piracetam is known to modulate neurotransmission in cholinergic and glutamatergic systems, to display neuroprotective and anticonvulsant properties, and to improve neuroplasticity. At the vascular level it appears to enhance microcirculation by reducing platelet activity and improving the deformability of red blood cells, which reduces the adhesion of damaged erythrocytes to vessel walls.

Piracetam is rapidly and almost completely absorbed after oral administration, with oral bioavailability close to 100 percent, peak plasma concentrations typically reached within 30 to 40 minutes, and a terminal elimination half-life of roughly five to six hours. It is excreted unchanged in the urine, with urinary excretion accounting for more than 98 percent of the administered dose, and it crosses the blood–brain barrier, preferentially accumulating in the grey matter of the cerebrum and cerebellum, the caudate nucleus, the hippocampus, the lateral geniculate body, and the choroid plexus. Despite this long clinical history, the study’s authors note that pharmacokinetic data in the Chinese population were limited, which motivated the present comparison conducted under the National Medical Products Administration’s regulatory framework.

The trial enrolled 56 healthy Chinese participants, 28 for a fasting study and 28 for a fed study, each conducted between November 26 and December 30, 2021. In each arm, participants were randomly assigned at a one-to-one ratio to receive either the test formulation, manufactured by China Resources Double-Crane Limin Pharmaceutical Co., Ltd in Jinan, or the reference product Nootropyl, manufactured by UCB Pharma SA, first, with a seven-day washout period separating the two single-dose treatments. Each dose consisted of one 800-milligram tablet taken orally with 240 milliliters of warm water. Participants in the fed arm received a standard high-fat breakfast 30 minutes before dosing. Blood samples were drawn at dense time points over 24 hours after administration, and plasma piracetam concentrations were quantified with a validated high-performance liquid chromatography–tandem mass spectrometry method with a lower limit of quantification of 100 nanograms per milliliter.

The bioequivalence analysis focused on three primary parameters: the maximum plasma concentration (Cmax), the area under the concentration–time curve from time zero to the last measurable concentration (AUC0–t), and the area under the curve extrapolated to infinity (AUC0–∞), all calculated by non-compartmental analysis. Under fasting conditions, the 90 percent confidence intervals of the geometric mean ratios of test to reference were 98.53 percent for Cmax, 98.40 percent for AUC0–t, and 98.49 percent for AUC0–∞. Under fed conditions, the corresponding intervals were 99.31 percent, 99.03 percent, and 99.01 percent. All values fell comfortably within the predefined acceptance range of 80.00 to 125.00 percent, the criterion regulators use to declare two oral formulations bioequivalent.

The underlying pharmacokinetic numbers tell a consistent story. In the fasting study, Cmax values for the test and reference formulations were 31,678.6 and 32,117.9 nanograms per milliliter respectively, with AUC0–∞ values of 159,790.6 and 162,479.2 nanogram-hours per milliliter and terminal half-lives of 5.8 hours in both groups. In the fed study, Cmax fell to 20,442.9 and 20,775.0 nanograms per milliliter for test and reference, while AUC0–∞ values were 149,739.0 and 150,995.7 nanogram-hours per milliliter and half-lives remained at 5.9 hours. Analysis of variance found no formulation effect on any of the three primary parameters in either study, although some sequence and period effects reached statistical significance for the AUC measures, a pattern the crossover design is built to accommodate.

One of the study’s most practically relevant observations concerns food. A high-fat meal reduced the rate of piracetam absorption without materially changing the total amount absorbed: Cmax dropped by roughly 35 percent and the time to maximum concentration shifted from about 0.5 hours to about 1.7 hours, regardless of which formulation was taken. The authors attribute this to the known physiology of fat-rich meals, which slow gastric emptying and prolong the drug’s residence in the stomach, delaying its delivery to the small intestine where most absorption occurs. Because piracetam is highly soluble and almost completely absorbed, total exposure as measured by AUC was not substantially affected. The researchers note that this may matter in situations where a rapid onset of peak concentration is desired, even though the drug may be administered with or without food.

Safety findings supported tolerability in this healthy volunteer population. In the fasting arm, five adverse events were reported in two participants, all associated with the reference product and all graded as mild, including anemia and transient blood count changes judged unrelated to the study drug. In the fed arm, five adverse events occurred in five participants, an incidence of 17.86 percent, comprising increases in blood uric acid, positive urine leukocytes, decreased leukocyte counts, and anemia, all mild and all resolved without treatment. No serious adverse events or suspicious unexpected serious adverse reactions occurred in either study, and the investigators concluded that the safety and tolerability profiles of the test and reference formulations were comparable.

The study also carries methodological strengths worth noting. Intra-subject variability was low, with coefficients of variation for Cmax of 17.93 percent under fasting conditions and 13.10 percent under fed conditions, and even lower values for the AUC parameters, indicating that piracetam is not a highly variable drug. The statistical power for all three primary parameters approached 100 percent, confirming that the sample size of 28 per arm was sufficient. The bioanalytical method was fully validated for selectivity, precision, accuracy, recovery, and stability, meeting regulatory acceptance criteria, and the trial was registered with the Chinese Drug Trial Registration platform under identifier CTR20213027.

The authors are candid about the limits of what a single-dose crossover study in healthy volunteers can show. The relatively small sample restricts generalizability to patients with comorbidities or those in specific age groups, and the single-dose design does not address long-term or repeated dosing. The pharmacokinetics of piracetam in special populations, including the elderly, patients with renal or hepatic impairment, and people on multiple medications, remain unclear and warrant further investigation. No comparison with other formulations or dose-ranging exploration was performed, and safety was assessed only within the trial period, leaving long-term safety data insufficient. Within those boundaries, however, the trial supports a straightforward regulatory conclusion: the generic piracetam tablet is bioequivalent to Nootropyl in rate and extent of absorption under both fasting and fed conditions in healthy Chinese participants, a result the authors suggest could help relieve the economic burden of treatment for Chinese patients.

Subject of Research: A bioequivalence and pharmacokinetics study of a generic piracetam tablet versus Nootropyl in healthy Chinese participants under fasting and fed conditions.
