Fetuin-A Reduction with Pioglitazone Eases Brain Inflammation in Aged Rats with Liver-Related Cognitive Impairment

Minimal hepatic encephalopathy, a subtle neurological complication of chronic liver disease, is marked by mild cognitive impairment and neuroinflammation, and it is particularly relevant in the aging population. A new research article published in the journal Biogerontology on 24 October 2026 reports that elevated hippocampal Fetuin-A expression was observed in aged rats with experimentally induced minimal hepatic encephalopathy, and that pharmacologically reducing this protein with pioglitazone was accompanied by reduced inflammatory signaling and recovered recognition memory. The work, led by Vishal Vikram Singh and colleagues with Papia Acharjee, focuses on a thioacetamide-induced model of the condition in old male rats.

The researchers administered thioacetamide at 50 mg/kg for 14 days to induce minimal hepatic encephalopathy in the aged animals, then treated them with pioglitazone at 3 mg/kg for 7 days. Western blot and immunofluorescence analyses revealed elevated hippocampal Fetuin-A expression in the affected rats, together with activation of the TLR4/MyD88/NF-κB signaling pathway and increased levels of the inflammatory cytokines IL-6 and TNF-α. These molecular changes are consistent with the neuroinflammatory state that has been associated with the condition in the aging brain.

After pioglitazone treatment, Fetuin-A expression was significantly reduced, and the reduction was accompanied by lower levels of TLR4, MyD88 and nuclear NF-κB, as well as decreased astrocyte activation.

The structural consequences of the treatment were assessed with histological and Golgi-Cox staining, which showed restoration of neuronal density, dendritic arborization and spine density in the hippocampus of the pioglitazone-treated old rats with minimal hepatic encephalopathy.

Behavioral analyses confirmed the molecular and histological findings, with the treated animals showing recovered recognition memory. Taken together, the study indicates that elevated hippocampal Fetuin-A is associated with TLR4/MyD88/NF-κB activation in aged rats with minimal hepatic encephalopathy, and that its pharmacological reduction by pioglitazone is accompanied by reduced neuroinflammation, restored hippocampal cytoarchitecture and recovered recognition memory.

The authors are careful to note an important limitation: a causal role for Fetuin-A remains to be established by direct perturbation. In other words, the study shows that lowering Fetuin-A with pioglitazone coincides with the observed improvements, but it does not yet demonstrate that Fetuin-A itself drives the neuroinflammatory cascade, since the intervention was pharmacological rather than a direct manipulation of the protein. The findings are also limited to an animal model of induced liver dysfunction in aged male rats and have not been tested in humans.

Within those constraints, the work identifies Fetuin-A as a candidate therapeutic target in the management of minimal hepatic encephalopathy in the aging population, a condition whose molecular mechanisms in old age remain poorly understood.

Subject of Research: The role of Fetuin-A in TLR4-mediated neuroinflammation during minimal hepatic encephalopathy in aged rats.
