Behavior Change Wheel Scoping Review Finds Child Nutrition Interventions Skew Toward High-Income Settings

A scoping review published in BMC Nutrition on 23 October 2026 has mapped how researchers use the behavior change wheel (BCW) when designing, implementing, and evaluating interventions aimed at improving child nutrition. The review, led by Jean Providence Nzabonimpa and Arnd Hofmeister, found that while the framework is widely applied, its use is concentrated in high-income countries and focused on a narrow set of outcomes, leaving major nutrition problems such as stunting and micronutrient deficiencies largely unaddressed.

Child malnutrition undermines holistic development and contributes to premature death and developmental disability. Although preventable, it persists, and the review’s authors point to suboptimal nutrition interventions as a continuing reason. Mounting evidence, they note, indicates that nutrition interventions informed by sound theoretical frameworks are effective in improving child nutrition outcomes, which makes it important to understand exactly how such frameworks are being used in practice.

The review set out to map the coverage of the BCW underlying the design, implementation, and evaluation of child nutrition interventions, asking which parts of the framework researchers actually deploy and in what contexts.

To do this, the authors followed the Preferred Reporting for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. They searched seven electronic databases using the Population, Concept, and Context (PCC) framework. Eligible studies were peer-reviewed, English-language articles published from January 2016 to June 2024 that applied the BCW in nutrition interventions for children under 18 years of age. Data from eligible articles were extracted and analyzed through thematic content analysis.

The initial search yielded 2,460 articles. After deduplication and application of the inclusion criteria, 22 articles made it into the final review.

Three main patterns emerged from the analysis. First, BCW intervention functions are more commonly used than policy categories are, suggesting that researchers gravitate toward the framework’s action-oriented layer rather than its policy-oriented one. Second, most articles explicitly use the COM-B model, but the behavior to change is not explicit. Third, all interventions were implemented in stable, high-income contexts, with one exception reported in a low- or middle-income country.

The review also found that most articles converge toward describing the BCW as the most comprehensive framework available for this purpose. However, the authors conclude that the evidence base offers little illumination of the differential contributions of each model component. In other words, while researchers agree the wheel is comprehensive, the published studies rarely clarify which parts of it are doing the work in any given intervention.

The geographic and thematic concentration is a central concern of the review. The authors highlight a predominant focus on contexts in high-income countries and on outcomes such as overall nutritional improvement and obesity prevention, whereas interventions addressing stunting, wasting, and micronutrient deficiencies remain scarce.

As a scoping review, the study maps existing literature rather than testing whether any intervention works, and its findings are limited to English-language, peer-reviewed articles published within the specified window. The article was published under open access and is currently available as a version subject to further edits before the final Version of Record appears.
