Retrieval Augmentation Improves Diabetes Language Models but Shows Smaller High-Complexity Gains

Retrieval-augmented large language models achieved higher semantic similarity scores and were generally preferred by clinicians over standard baseline models in post-diagnosis diabetes question-and-answer evaluations. Researchers evaluated system performance by developing the DiabMedQA dataset, testing retrieval-augmented models across four health management tasks and three levels of question complexity to assess response quality.

Across the semantic evaluations, the Qwen2.5-14B model performed best, recording an average BERTScore improvement of 10.56% after implementing retrieval-augmented generation. While a combined analysis demonstrated significant BERTScore improvements across all question complexity tiers, performance gains were significantly smaller for higher-complexity inquiries, and improvements on high-complexity questions reached statistical significance in only one of the four evaluated models.

Task-specific analysis revealed that diet advice tasks yielded the lowest performance, with retrieval-augmented language model BERTScores falling below 0.8. In clinician evaluations, practitioners preferred retrieval-augmented Qwen2.5-14B answers over baseline outputs, indicating that while retrieval augmentation provides measurable value in diabetes management questions, question complexity remains a critical factor governing model optimization.
