A New Mechanism-Based Framework Aims to Untangle Drug-Induced Tremor

Drug-induced tremor is far more than a single side effect. In a new Perspective article published in Frontiers in Neurology, neurologist Hae-Won Shin of Chung-Ang University in Seoul proposes a mechanism-based framework intended to bring clinical order to a group of syndromes that differ widely in cause, course, and treatment needs.

The core idea is simple but consequential: instead of treating drug-induced tremor as one entity, clinicians should integrate the tremor phenotype, the pharmacology of the offending drug, associated neurological findings, and the clinical course into a single structured assessment.

The framework organizes initial evaluation around three mechanistic categories. The third covers systemic or neurotoxic effects, where tremor may signal broader harm rather than a benign exaggeration of normal tremor circuits.

Shin acknowledges that not every culprit drug fits neatly into these buckets. For agents such as valproate and serotonergic antidepressants, the mechanisms are less clearly understood, so the framework explicitly allows drug-specific assessment rather than forcing an uncertain fit.

These mechanistic considerations are not academic labels; they are meant to guide action. The proposed approach links each assessment directly to treatment adjustment and to targeted investigation, so that the mechanism a clinician suspects shapes what is changed and what is tested.

Associated neurological findings carry particular diagnostic weight. Cerebellar signs, neuropathic features, cognitive changes, or systemic findings should prompt evaluation for neurotoxicity and metabolic abnormalities, according to the framework, rather than being dismissed as incidental.

A separate pathway addresses tremor accompanied by parkinsonian features. Here the article recommends clinical follow-up to determine whether the syndrome recovers, persists, or progresses over time.

The framework also directs attention to tardive tremor, a delayed complication of prolonged drug exposure. Assessment should weigh its distinctive clinical features together with the timing of symptoms in relation to long-term exposure and withdrawal, since that temporal pattern is central to recognizing the syndrome.

Shin positions the proposal as a complement to, not a replacement for, the Movement Disorder Society tremor classification. By connecting mechanistic assessment to investigation, treatment, follow-up, and patient counseling, the framework aims to offer a practical route through the heterogeneity that has long made drug-induced tremor difficult to teach, study, and manage.

One caveat is stated plainly in the article itself: the framework is a proposal, and prospective validation is needed to determine whether it improves clinical outcomes.
