Stanford Trial Finds Safe Treatments Enable Kids With Food Allergies to Eat

About a third of children with multiple food allergies may be able to safely eat full portions of the foods that trigger their reactions, according to a Stanford Medicine-led clinical trial reported in JAMA Pediatrics. The study explores whether shifting immune responses can move patients from strict avoidance toward regular, diet-based exposure.

Food allergies affect a substantial share of the population, and reactions can be dangerous or life-threatening. For children, managing risk is complicated because common allergens—such as peanuts, wheat, milk, and eggs—are widespread in everyday foods. Many children react to more than one trigger, turning school days and social events into constant risk assessments.

The research follows a nationwide, multi-stage trial of omalizumab, a biologic antibody therapy. Omalizumab binds circulating IgE, reducing the activity of allergen-specific antibodies that drive many allergic pathways. In 2024, the FDA approved omalizumab to help protect against reactions from accidental exposure to small amounts of food allergens.

In the new phase (stage 2), investigators asked a more ambitious question: can omalizumab—used alone or combined with oral immunotherapy—enable children to tolerate full servings of multiple allergy-causing foods? Tolerance was defined rigorously: participants needed to consume full 2,000 mg servings of three different trigger foods by the end of the study.

The trial enrolled 117 children with peanut allergy plus at least two additional food allergies, with a median age of 7. One treatment path used omalizumab throughout the year, while a second path used omalizumab for eight weeks before transitioning to oral immunotherapy for the remainder.

Completion and adherence shaped outcomes. In the omalizumab-only group, 36% of participants achieved the primary tolerance goal. In the group that added oral immunotherapy, 19% met the same standard after a year. Researchers found that more participants discontinued the combined approach—often due to adverse effects—limiting who could fully receive the intended therapy.

Importantly, among families able to complete the combined regimen, success rates were comparable to omalizumab alone. This suggests that the “best” strategy may depend less on biology alone and more on feasibility, side-effect tolerance, and family capacity to maintain the daily routine.

The findings support a growing view that food-allergy care can be tailored. Some patients seek protection mainly against accidental exposures; others aim for diet expansion. Clinicians may use these results to match treatment intensity to individual goals and constraints.
