Researchers Outline a Universal Standard to Tackle Stigma Across Healthcare

In a perspective article published in Harm Reduction Journal, the group describes the development of The Stigma-Free Standard, an online intervention package designed to be used across a range of health services, and presents the program logic that underpins it.

The authors hypothesise that stigma reduction efforts can usefully proceed on that assumption, rather than waiting for services to identify which stigmatised conditions, identities or practices are present in their patient population.

The rationale is grounded in well-documented harms. According to the paper, stigma can result in restricted, denied or sub-standard care. People living with stigmatised conditions, identities or practices may avoid or delay healthcare because of well-founded concerns about how they will be treated, and this avoidance contributes to poorer health outcomes and health inequity. The authors note that efforts to reduce stigma in healthcare face many challenges, including feasibility, because of the diversity, complexity and breadth of health-related stigmas.

That breadth is the core problem the new standard is designed to address. The Stigma-Free Standard instead moves away from a condition-by-condition approach, using a new conceptualisation of stigma that cuts across conditions, identities and practices. The intervention is delivered online, which the authors say is intended to make implementation in health services feasible and sustainable.

The prototype was not built from scratch. The paper states that its development drew on the existing literature, foundational research and co-design, although the perspective article does not report outcome data from implementation. The authors are explicit that what they are presenting is the program logic that underpins The Stigma-Free Standard, and that the paper aims to provide a transparent and robust account of the new intervention.

Evaluation of the initial implementation is planned, and the authors frame the program logic as a resource for that work. They state that the paper aims to provide a transparent and robust account of the new intervention to support the interrogation of findings from impending evaluations, and to serve as a resource for future stigma reduction efforts. In other words, the standard’s effectiveness has not yet been demonstrated; the current article is a methodological and conceptual foundation for testing that will follow.

The authors acknowledge the scale of the undertaking. “A new conceptualization of a ‘universal’ approach to stigma reduction for implementation in health services is a significant undertaking,” they write in the conclusion, positioning the paper as an accountability document as much as a description. By publishing the program logic openly, the team invites scrutiny of the assumptions behind the intervention before and during evaluation, rather than after results are reported.

The article is published as a perspective piece and is open access, with the version released in October 2026 subject to further editorial edits before the final version of record. Whether that model delivers measurable improvements in care will depend on the evaluations the authors say are now impending.
