A cross-sectional study examining discussions on the social media platform Reddit found that side effects represented the most frequent theme among users discussing tamoxifen, accounting for 45 percent of analyzed content.

Tamoxifen is a key component of adjuvant therapy for hormone receptor–positive breast cancer, but its associated side effects can impair quality of life and adherence to treatment regimens. Because patients increasingly look for experiential information online, Reddit has emerged as a common platform for discussing tamoxifen, prompting the authors to systematically evaluate the content and sentiment of these public exchanges.

To conduct the analysis, investigators extracted posts and comments referencing “tamoxifen” from the r/breastcancer subreddit spanning 2021 to 2025. The collected text was de-identified, thematically coded, and evaluated using Bing sentiment scoring alongside NRC emotion categorization. In total, the researchers analyzed 194 posts and 6,013 comments from the online forum.

Thematic analysis showed that side effects were the predominant focus at 45 percent, followed by discussions focused on support at 17 percent and advice seeking at 12 percent. Aggregated comment sentiment scores were more negative within comment posts and became less negative over time, although the authors noted that these sentiment scores were not normalized by word count. Across emotional categories, fear, anticipation, and sadness were identified as the most common emotions present in the text.

The study concluded that Reddit discussions reflect patients’ prominent concerns and lived experiences while undergoing tamoxifen therapy. The authors noted that understanding the concerns patients prioritize in online communities may help clinicians anticipate patient needs, better address symptom burden, improve communication, and align counseling with survivorship needs during long-term endocrine therapy.
