Older Patients May Fare Better After Trigeminal Neuralgia Surgery, Study Finds

Patients older than 60 who underwent microvascular decompression for classic trigeminal neuralgia showed better long-term pain control than younger patients in a retrospective study published in the European Journal of Medical Research. At a mean follow-up of 35 months, 82.3% of patients in the older group had good pain control, defined as Barrow Neurological Institute grades I–II, compared with 56.3% of patients aged 60 or younger, a difference the authors report as statistically significant (P = 0.040).

Of these, 85 patients were aged 60 years or younger and 50 patients were older than 60.

Beyond the age comparison, the analysis identified other factors linked to pain outcomes. Univariate regression analysis showed that age, preoperative drug responsiveness, and intraoperative arterial compression were significantly associated with good pain control. In multivariate regression analysis, only age and intraoperative arterial compression remained significantly correlated with good pain control, suggesting that these two factors independently predicted how patients fared after surgery.

The authors conclude that for some patients with trigeminal neuralgia over 60 years of age, microvascular decompression may achieve a good pain prognosis.
