Circulating cell-free DNA shows strong diagnostic and prognostic signal in ovarian cancer study

Circulating cell-free DNA measured in the blood distinguished women with ovarian cancer from those with benign ovarian lesions and from healthy controls with high accuracy in a prospective case-control study published in BMC Cancer. The research, led by Gamalat A. Elgedawy and colleagues, set out to test whether circulating tumor cell-free DNA could serve as a non-invasive biomarker for ovarian cancer, a disease the authors describe as lacking efficient screening methods or trustworthy biomarkers with high sensitivity and specificity.

The study enrolled 150 participants: 50 patients with confirmed ovarian cancer, 50 patients with benign ovarian lesions, and 50 controls. For the ovarian cancer patients, the researchers performed medical, laboratory, and imaging evaluations together with survival analysis. Quantification of cell-free DNA was carried out by real-time PCR.

The mean cell-free DNA concentration differed markedly between the groups. The malignant group averaged 11.84 ng/mL, compared with 3.90 ng/mL in the benign group and 0.23 ng/mL in the control group. According to ROC curve analysis, cell-free DNA had the highest value as a discriminatory marker for predicting ovarian cancer in suspected cases, with an AUC of 0.934 (p < 0.001, 95% CI: 0.881–0.986). It also showed strong performance for predicting late stage, with an AUC of 0.993 (p < 0.001, 95% CI: 0.978–1.0), and was described as a reasonable predictor of mortality, with an AUC of 0.814 (p = 0.001, 95% CI: 0.690–0.937).

Beyond diagnosis, the biomarker appeared to carry prognostic information. Multivariate logistic regression reinforced the independent prognostic value of cell-free DNA for progression-free survival and overall survival. The authors report that their results indicate cell-free DNA has a strong correlation with tumor grade and predictive capability in Cox models, suggesting it may serve as a diagnostic and prognostic biomarker for ovarian cancer.

In a clinical setting, the authors write that these findings support the use of cell-free DNA in ovarian cancer risk stratification and treatment planning, potentially enabling aggressive therapies for patients with high cell-free DNA expression. The investigators believe that circulating tumor cell-free DNA can provide crucial clues concerning both primary and metastatic ovarian cancers, which motivated the study’s goal of identifying its potential as a non-invasive biomarker.

The findings come with important caveats. The study was a single case-control investigation of 150 participants, and the reported accuracy figures reflect discrimination within this cohort rather than validated performance in a general screening population. The mortality prediction result, with an AUC of 0.814, is described by the authors as reasonable rather than definitive, and the confidence intervals span a range that leaves room for uncertainty. As with any case-control design, the results establish associations and discriminatory capacity within the studied groups; they do not by themselves demonstrate that cell-free DNA testing improves outcomes when deployed in routine care.

Nevertheless, the study adds to the case for looking beyond CA-125, the long-standing biomarker referenced in the article’s title, toward circulating cell-free DNA as a complementary tool. The authors’ conclusion positions the marker as potentially useful both for identifying ovarian cancer among suspected cases and for stratifying patients by risk, directions that would require further validation before clinical adoption.
