Salivary gland carcinomas are a heterogeneous group of malignancies in which B7-H3/CD276 expression across histologies and cellular compartments remains incompletely characterized. A new study examines B7-H3, also known as CD276, an emerging therapeutic target in solid tumors and the focus of antibody-based agents such as antibody-drug conjugates. Using immunohistochemistry on tissue microarrays, researchers scored cytoplasmic and membranous B7-H3 staining separately in 72 malignant salivary gland carcinoma cases and 14 normal salivary gland specimens, 11 of which were matched tumor–normal pairs from the same patients.
The central finding concerns compartment specificity. Cytoplasmic B7-H3 staining was evaluable in 69 tumors and proved markedly higher in carcinomas than in normal salivary tissue. In unpaired comparisons, the median cytoplasmic H-score was 56.3 in tumors versus 2.5 in normal tissues, a difference that remained significant after correction for multiple testing (p = 0.0006; q = 0.0023). When the analysis was restricted to the 11 matched tumor–normal pairs, the contrast persisted: a median H-score of 70.0 in tumors against 2.5 in adjacent normal tissue (p = 0.0029; q = 0.0059). The matched-pair analysis indicates the cytoplasmic elevation was present within the same individuals, not only when different patients were compared.
Membranous staining told a different story. It was generally weaker than cytoplasmic staining across the cohort, and tumor–normal differences in the membrane compartment did not reach statistical significance in either the unpaired or the matched analysis. This distinction matters because B7-H3 is an emerging therapeutic target in solid tumors and the focus of antibody-based agents such as antibody-drug conjugates.
The exploratory analyses added a further layer of nuance. In pooled analyses across the cohort, membranous H-scores showed modest positive correlations with ordinal T category (ρ = 0.292; p = 0.016; q = 0.099) and with the Ki-67 proliferation index (ρ = 0.306; p = 0.015; q = 0.099). Tumors with higher membranous B7-H3 scores tended, on average, to present with higher T categories and higher proliferation indices. However, after adjustment for multiple comparisons, neither association retained formal statistical significance, and when mucoepidermoid carcinoma and adenoid cystic carcinoma were analyzed separately, neither correlation held up. The authors therefore treat these findings as hypothesis-generating rather than established clinical signals.
The study was conducted on tissue microarrays containing 72 malignant salivary gland carcinoma cases and 14 normal salivary gland specimens, including 11 matched tumor–normal pairs. The authors explicitly state that the potential value of B7-H3 as a biomarker requires evaluation in larger, subtype-specific cohorts that include outcome and treatment-response data.
What the study does establish is that cytoplasmic B7-H3 staining provides the strongest tumor-versus-normal discrimination among the compartments examined. This supports the idea that B7-H3 is upregulated in salivary gland carcinoma relative to normal salivary gland tissue, consistent with its emerging role as a target in solid tumors more broadly. But the same data caution against reading any B7-H3 stain as equivalent to any other. The authors conclude that B7-H3 staining in salivary gland carcinomas differs between cytoplasmic and membranous compartments and across histologic subtypes, and that the clinical significance of membranous staining remains uncertain.
In this cohort, cytoplasmic staining distinguished tumors from normal salivary tissues, whereas membranous staining was generally lower and showed no significant tumor–normal difference. Whether membranous B7-H3 levels correlate with response to anti-B7-H3 agents remains an open question that this study was not designed to answer.
The authors state that the potential value of B7-H3 as a biomarker requires evaluation in larger, subtype-specific cohorts with outcome and treatment-response data.
