Antidepressant Discontinuation Research Shifts From Case Reports to Tapering Strategies

Researchers have mapped a quarter-century of scientific literature on withdrawal from two commonly prescribed antidepressants, paroxetine and venlafaxine, and found that the field has moved decisively from documenting symptoms to designing ways of stopping the drugs more safely. The new bibliometric analysis, published in Frontiers in Medicine, draws on records collected from the Web of Science and Scopus databases covering the years 2000 to 2025.

Both medications are known to cause discontinuation symptoms at least partly because of their shorter half-lives, meaning they are cleared from the body relatively quickly. Despite long-standing clinical awareness of this problem, the authors note that no thorough macroscopic analysis of the historical development and structure of this research area had previously been carried out.

The analysis included a total of 1,313 documents. Annual publication output peaked around 2007 and has remained stable since then, according to the study. Geographically, the publications were overwhelmingly concentrated in the West, with the United States standing out as the leading contributor among North American and European countries.

Keyword and thematic analyses showed that the clinical manifestations reported as “anxiety” and “withdrawal” remain basic themes in the field, indicating that these symptom descriptions continue to anchor the literature. At the same time, the authors observed an interesting shift in research emphasis: work is now moving away from case reports of early phenomenology toward “tapering” and “pharmacokinetics,” which they describe as the underlying strategies for managing discontinuation.

Co-citation networks constructed from the Web of Science data revealed a persistent theoretical influence from pioneering psychopharmacologists and early specialized studies, suggesting that the intellectual foundations of the field have remained remarkably durable even as its practical focus has evolved. In their conclusions, the researchers characterize the trajectory of venlafaxine and paroxetine withdrawal research as an evolution from clinical understanding to mechanistic investigation and protocol creation.

Looking forward, the authors suggest that embedding tapering ideas within a general treatment paradigm may offer a potential pathway to support clinical outcomes, a framing they present as the main route forward for optimizing care. As a bibliometric study, the analysis maps patterns in published research rather than evaluating the effectiveness of any particular tapering protocol in patients, and its conclusions reflect the structure of the literature itself rather than new clinical trial findings.
