Adult H3 K27-Altered Diffuse Midline Glioma Shows Striking Imaging and Survival Heterogeneity in Small Case Series

A single-center case series from the Medical College of Wisconsin has documented the widely varying imaging trajectories and survival outcomes of adults with H3 K27-altered diffuse midline glioma, an aggressive tumor of the brain's central structures that is far better characterized in children than in adults. The study, published in Frontiers in Oncology, combined a descriptive review of the literature with a retrospective analysis of six adult patients whose diagnoses were confirmed and who were assessed at autopsy, offering one of the more detailed longitudinal looks at how these rare tumors evolve on magnetic resonance imaging. Diffuse midline glioma with H3 K27 alterations is a malignant central nervous system tumor with limited treatment options and poor outcomes.

Although the entity is more commonly diagnosed in children, adult cases are increasingly recognized, yet they remain under-characterized, particularly with respect to how tumors change over time on serial imaging and how they respond to multimodal treatment. The Wisconsin team set out to address that gap by tracking volumetric MRI measures across the course of each patient's disease. To do so, the researchers included only patients aged 21 years or older with a confirmed diagnosis of H3 K27-altered diffuse midline glioma.

At every available imaging timepoint, they manually segmented two features: contrast-enhancing regions on T1-weighted post-contrast scans and hyperintense areas on FLAIR sequences, which capture infiltrative tumor and surrounding signal change. From these volumes they calculated a contrast enhancement ratio, defined as enhancing volume divided by FLAIR volume, to quantify the balance between a tumor's enhancing, blood-brain-barrier-disrupting component and its infiltrative burden. Treatment timelines were then retrospectively aligned with the imaging schedule, allowing the investigators to relate therapeutic interventions to radiographic progression and response.

The six-patient cohort displayed marked radiographic heterogeneity, with no single uniform pattern of growth or treatment effect emerging across the group. Notably, periods of FLAIR signal reduction were observed to occur around the time of bevacizumab treatment, a finding the authors highlight as a recurring feature in their imaging timelines. The lowest mean values were seen in Patient 5 at 0.081 and Patient 1 at 0.225, indicating tumors dominated by non-enhancing, infiltrative signal rather than discrete enhancement.

Patient 4 occupied an intermediate position with a ratio of 0.312, while Patient 3 showed the highest mean ratio at 0.668, reflecting a tumor in which enhancing tissue made up roughly two-thirds of the total FLAIR-defined volume. These differences underscore how differently the same molecularly defined tumor can present and evolve on imaging in adults. The most striking divergence in the series involved the ATRX gene.

Two patients carried ATRX mutations and survived comparatively brief periods of 26 days and 164 days, while the four ATRX-wild-type patients survived between 728 and 2,299 days. Although the numbers are far too small to establish a definitive prognostic relationship, the authors conclude that, within their cohort, ATRX-mutant cases showed comparatively short survival, a signal they suggest warrants attention in future studies of adult H3 K27-altered diffuse midline glioma. Beyond the genetic findings, the study carries a cautionary message about imaging itself.

The authors emphasize the limitations of imaging-based modalities for tracking tumor progression in this disease, noting that volumetric measures of enhancement and FLAIR signal may not fully capture the true extent or biological behavior of infiltrative glioma tissue. The availability of autopsy confirmation in their cases allowed the radiographic observations to be framed against the ultimate pathological reality of each tumor, reinforcing the point that what MRI shows is an imperfect proxy for disease burden. The retrospective, single-center design and the cohort of just six patients are important limitations.

Survival figures and enhancement ratios from such a small series cannot be generalized, and the observed association between ATRX status and survival requires validation in larger, ideally multi-institutional cohorts. Nevertheless, the work adds to a growing recognition that adult H3 K27-altered diffuse midline glioma is not a single radiographic or clinical entity.
