Modeling study estimates ovarian cancer risk after delayed BRCA1 preventive surgery

A modeling study published in JAMA Surgery estimates that women carrying a BRCA1 mutation who defer bilateral salpingo-oophorectomy until age 50 may face a substantial risk of developing ovarian cancer. The analysis also considered the residual ovarian cancer risk among older women in good health, a level the authors said was sufficiently high to consider preventive surgery.

The research focused on projected benefits of bilateral salpingo-oophorectomy by age in BRCA carriers, with particular attention to BRCA1 carriers. Rather than reporting outcomes from a randomized clinical trial, the study used a model to estimate how postponing surgery through age 50 could affect subsequent ovarian cancer risk. The supplied research summary does not provide the model’s full inputs, uncertainty estimates, eligibility criteria or individual clinical results.

The reported finding is specific to BRCA1 carriers who delay surgery until age 50. The study did not show that surgery prevents ovarian cancer in every carrier, and its projections should not be read as a universal recommendation about the timing of preventive surgery. Nor did the summary establish that the projected benefit differs among carriers beyond the age-based analysis described in the release.

Because the findings are model-based, they estimate potential benefit rather than demonstrate cancer prevention directly in a treated population. The results support considering age when evaluating bilateral salpingo-oophorectomy for BRCA1 carriers, while the evidence summarized here does not establish that every carrier should undergo surgery at a particular age. The study’s contribution is to project the consequences of deferral and age-related residual risk, not to provide direct clinical-outcome comparisons of surgery versus continued surveillance.
