Scoping review maps outcome measures in 22 recovery college evaluations

A scoping review of 22 recovery college effectiveness studies found that researchers used 39 distinct measure and indicator entries spanning intra-personal and psychological changes, behavioral and social changes, and system and environment. The most frequently reported measure was the Warwick-Edinburgh Mental Wellbeing Scale, including its short version, which appeared in 11 articles, or 50.0% of the included set. The Questionnaire about the Process of Recovery appeared in four articles, or 18.2%, while 28 entries appeared in only one article each.

The review mapped how outcomes were measured and reported; it did not determine whether recovery colleges improve health or educational outcomes. Recovery colleges are educational, recovery-oriented approaches in mental health. Their evaluations may differ in purpose and setting, which can shape the outcome indicators selected by investigators.

The researchers followed JBI scoping-review methodology and PRISMA-ScR reporting guidance when searching PubMed, Web of Science, CINAHL Ultimate, ERIC and seven databases accessed through ProQuest. Eligible records were English-language journal articles reporting quantitative recovery college evaluations, including quantitative components of mixed-methods studies. Eligible outcomes included questionnaire scores collected before and after recovery college participation and health-service indicators such as hospital admissions and service contacts.

Three reviewers independently screened titles and abstracts and assessed full texts. Data were extracted collaboratively by two reviewers and independently checked by a third, while measured constructs were classified through inductive content analysis. Of 336 records identified, 149 remained after deduplication and 22 articles were included.

WEMWBS was used in exactly half of the included articles, making it the most common individual instrument, although the review did not characterize the collection as having a uniform emphasis on psychological well-being. The 28 single-use entries show that many measures were confined to one evaluation, and direct assessment of educational processes and outcomes was comparatively limited in the literature reviewed. The review also assessed how authors supported the measurement tools they used.

Fifteen articles, representing 68.2% of the 22 included studies, cited previous psychometric information or identifiable validation literature. Two articles, or 9.1%, explicitly reported within-study reliability estimates. Six articles, or 27.3%, reported neither specific psychometric evidence nor an identifiable validation citation, while one article was not applicable to scale psychometrics.

These reporting patterns identify gaps in the available evidence; they do not demonstrate that any instrument was ineffective or unsuitable. The review itself was designed to map measurement choices rather than test the effectiveness of recovery colleges, so its results cannot establish which program produces better outcomes or which measures best predict success. The authors say the findings may inform outcome-selection and study-design decisions made jointly with learners and other stakeholders.

Involving participants can help align evaluation with the educational philosophy of recovery colleges while also ensuring that learners’ individual goals are represented. Further evaluation of existing instruments’ suitability for intended recovery college populations is therefore needed. Even so, the conclusions remain descriptive rather than causal, offering a structured account of current outcome-measurement practices without assessing whether recovery colleges themselves are effective.
