Molecular Testing Reshapes Diagnosis of Pediatric Bone Tumors

Pediatric bone tumors have long posed a diagnostic challenge for pathologists, but a new review published in Diagnostic Pathology argues that the field is undergoing a quiet transformation. The work, authored by Archana Shenoy and John D. Reith, describes how the evolution of molecular and diagnostic adjuncts has enabled diagnosis of these challenging lesions even when only limited diagnostic tissue is available, in a setting where pediatric bone tumors are increasingly sampled through image-guided biopsies.

The authors emphasize that radiologic correlation remains critical in the evaluation of these lesions. Because sampling quality can affect diagnostic interpretation, the review emphasizes that radiologic correlation remains critical in the evaluation of these lesions.

Among the most striking updates described in the review is the expanded understanding of giant-cell lesions. The review reports that understanding of giant-cell lesions has expanded with the identification of highly specific histone mutations in giant cell tumors and chondroblastoma.

Molecular testing has also played a clarifying role in other areas. According to the review, it has allowed validation of newer entities such as the xanthogranulomatous epithelial tumor, also described as keratin-positive giant cell tumor, while simultaneously clarifying the diagnostic features of well-defined lesions such as the aneurysmal bone cyst.

Underlying all of these advances is a practical concern that the authors place at the center of modern practice: tissue preservation. The review identifies the avoidance of harsh acid decalcification techniques as the mainstay for preserving nucleic acids in bone specimens. The review identifies avoidance of harsh acid decalcification techniques as the mainstay for preserving nucleic acids and enabling application of newer ancillary studies.

The review notes that the evolution of molecular and diagnostic adjuncts has enabled diagnosis of these challenging lesions with limited diagnostic tissue, while tissue preservation remains essential for applying newer ancillary studies.

The authors position the review as an overview of the current diagnostic approach to pediatric bone tumors, with key updates intended to guide practicing pathologists.
