Daily symptom tracking reveals intermittent dementia behaviors in Swedish nursing homes

Behavioral and psychological symptoms of dementia (BPSD) are often assessed with retrospective questionnaires covering weeks or months, leaving everyday symptom variation in routine care poorly documented. A Swedish study published in BMC Geriatrics took a different approach: care staff rated twelve common symptoms at the end of every work shift using a digital tool adapted from the Neuropsychiatric Inventory Nursing Home version, generating more than one million symptom registrations across a median observation period of 270 days per resident.

The study followed 230 individuals with dementia in six nursing homes across six Swedish municipalities. Because staff recorded symptom severity on a scale from 0 to 3 at every shift, the researchers could describe prevalence, severity, co-occurrence, differences between units, and patterns across shifts and weekdays at a daily resolution that periodic assessments are not designed to provide. Analyses were descriptive and person-weighted, giving each individual equal weight regardless of how long they were observed.

The cumulative picture differed sharply from the day-to-day one. Between 70 percent (euphoria) and 90 percent (agitation) of residents showed each symptom at some point during observation, yet each symptom’s mean daily prevalence was far lower, ranging from 4 to 18 percent, and recorded symptoms were predominantly of low severity. At least one symptom was documented on a median of 43.5 percent of registered days, with wide variation between individuals, indicating an intermittent rather than persistent symptom landscape.

Symptoms that are clinically related tended to occur together, a pattern consistent with the established domain structures of the NPI-NH. The mean number of symptoms per resident per day varied more than twofold across the six units, pointing to substantial between-unit heterogeneity that averaged assessments can obscure.

Timing mattered as well. Symptoms were most frequent on evening shifts and least frequent at night, with sleep disturbances the notable exception, while there was little variation across weekdays. The authors note a practical limitation: just under half of all possible shift registrations were completed, so the dataset reflects documented rather than fully continuous observation. Even so, the findings offer an empirical baseline for future multilevel analyses and support monitoring approaches that track both individual symptoms and the daily number of symptoms in nursing home care.
