Clinician education program improves pain outcomes in long COVID care trial

The trial, registered as NCT05167227, tested the Long COVID and Fatiguing Illness Recovery Program. Long COVID is a heterogeneous chronic condition whose symptoms often overlap with other infection-associated chronic conditions, a clinical similarity the researchers say exposes gaps in healthcare education and delivery and leaves many primary care clinicians unprepared to manage these patients.

From December 2021 through October 2023, the study randomized 20 primary care clinicians in a 1:1 ratio. Clinicians in the intervention arm participated in weekly multidisciplinary Extension for Community Healthcare Outcomes (ECHO) case-based virtual community of practice sessions, alongside monthly webinars and quarterly short courses. Those in the control arm received the monthly webinars and quarterly short courses but not the weekly ECHO sessions.

A total of 590 patients were assigned to the participating clinicians and completed surveys using the Patient-Reported Outcomes Measurement Information System (PROMIS)-29 instrument at baseline and at 3, 6, 9, and 12 months. The analysis included completed surveys from 298 control patients and 289 intervention patients, examined with statistical tests and linear mixed-effects regression to assess both between-arm differences and longitudinal treatment effects.

Both arms improved over the 12 months on multiple PROMIS scales, including Fatigue, Physical Function, Pain Interference, Social Roles, Dyspnea, and Applied Cognition, with scores approaching reference population norms by the end of the study.

The clearest between-arm difference appeared on pain interference. At 12 months, the intervention arm scored better than control, with a mean of 57 (standard deviation 11) versus 60 (standard deviation 10), a difference the authors report as statistically significant at P = 0.009.

Longitudinal analyses reinforced that finding. The estimated treatment effect on pain interference was −1.95, with a 95% confidence interval of −3.79 to −0.10 and P = 0.04. In addition, a higher proportion of patients in the intervention arm achieved clinically meaningful improvements, defined as at least 5 T-score points, compared with controls: 43% versus 29%, with P = 0.01.

The authors conclude that the ECHO case-based virtual community of practice model showed evidence that clinician-focused education may enhance primary care management of patients with complex fatiguing illnesses, supporting the model’s value in under-resourced settings. The trial was single-blind, and the reported outcomes are patient-reported measures rather than clinician-assessed or objective clinical endpoints.
