Therapeutic Alliance Dimensions Differently Shape Outcomes in Two Adolescent BPD Therapies

Early therapeutic alliance does not predict treatment outcomes uniformly across psychotherapies for adolescents with borderline personality disorder, according to a new study published in Borderline Personality Disorder and Emotion Dysregulation. The research, published on 3 October 2026 as an open-access research article, examined how specific dimensions of the alliance relate to outcomes in two distinct psychotherapeutic approaches.

The therapeutic alliance is a well-established predictor of treatment outcomes across age groups, mental disorders, and psychotherapeutic modalities. However, alliance is a multifaceted construct, and its specific dimensions, as well as their relevance across different treatment approaches, have rarely been examined in adolescents. The new study set out to address this gap by disentangling which aspects of the alliance matter, and for which therapy.

The study investigated the relationship between therapeutic alliance and treatment outcomes in a sample of treatment-seeking adolescents with subthreshold or full borderline personality disorder. The sample comprised 57 participants with a mean age of 15.9 years (standard deviation of 1.3 years). Participants received either dialectical behavior therapy for adolescents (DBT-A) or adolescent identity treatment (AIT).

The therapeutic alliance was assessed using Bordin’s model, which comprises three dimensions: task, goal, and bond. This framework allows researchers to distinguish between agreement on the tasks of therapy, agreement on its goals, and the emotional connection between therapist and patient, rather than treating alliance as a single undifferentiated score.

The results showed that early alliance was not uniformly related to outcome across the two treatments. While global alliance predicted improvement in personality functioning in AIT, it was not significantly associated with outcomes in DBT-A. This contrast suggests that the same overall measure of alliance may carry different informational weight depending on the therapeutic approach being delivered.

At the dimensional level, task agreement appeared most relevant within DBT-A, whereas emotional bond was positively associated with psychosocial functioning in AIT. In other words, in DBT-A, agreement between therapist and adolescent on what the work of therapy involves seemed to matter most, while in AIT, the quality of the emotional relationship itself was linked to better psychosocial functioning.

The authors also conducted exploratory multigroup comparisons, which provided the strongest support for a treatment-specific role of emotional bond in AIT. By contrast, the apparent relevance of task agreement within DBT-A was not accompanied by significant between-treatment differences, meaning the evidence for that dimension’s treatment-specific role was weaker.

In the study’s conclusion, the alliance–outcome association was described as not uniform, appearing to vary across treatment approaches and alliance dimensions. The findings suggest that emotional bond may be particularly relevant in AIT, while task agreement may play a more prominent role within DBT-A, although the latter effect was not supported by significant between-treatment differences.

The study was registered at clinicaltrials.gov under identifier NCT02518906, with a registration date of 10 August 2015. The article was published open access, and the version shared at publication is an accepted manuscript shared early to provide faster access to peer-reviewed research; it is citable and carries a permanent DOI, and is subject to further edits before the final Version of Record replaces it.

For clinicians and researchers working with adolescents with borderline personality disorder, the findings underscore that the alliance–outcome relationship cannot be assumed to operate identically across therapies. The study’s sample of 57 treatment-seeking adolescents, and its exploratory nature for the between-treatment comparisons, mean the results should be interpreted with appropriate caution, but they highlight the value of examining alliance dimensions separately rather than as a single global construct.
