Massive Gluten-Free Flour Inhalation Leads to Severe Bronchial Obstruction and Lung Atelectasis

A rare and severe case of acute hypoxemic respiratory failure in a previously healthy two-year-old boy was linked to the accidental inhalation of a large quantity of gluten-free flour powder, leading to complete lung atelectasis and life-threatening bronchial obstruction.

The patient presented with acute hypoxemic respiratory failure that was refractory to standard initial treatments. Despite receiving inhaled bronchodilators, systemic corticosteroids, and supplemental oxygen therapy, the child’s condition deteriorated rapidly, necessitating endotracheal intubation and invasive mechanical ventilation. Initial extensive diagnostic investigations were performed to rule out common infectious causes or conventional aspirated foreign bodies, but these efforts yielded no definitive common infectious etiology or typical aspirated material.

Chest imaging provided critical clues to the underlying pathology, demonstrating complete atelectasis of the left lung and atelectasis specifically in the right upper lobe. This pattern of lung collapse indicated a significant mechanical obstruction within the airways. The definitive diagnosis was established through flexible bronchoscopy, which, coupled with therapeutic bronchoalveolar lavage, revealed and allowed for the removal of abundant white, highly viscous endobronchial material. The successful removal of this material resulted in the progressive re-expansion of the affected lung tissue and marked clinical improvement for the child.

A meticulous review of the clinical history subsequently identified the likely precipitating event: the inhalation of gluten-free flour powder. This occurred after an accidental activation of a planetary mixer during cake preparation. This specific exposure—a massive inhalation of a fine powder—is hypothesized to have contributed to the severe airway obstruction through two primary mechanisms: direct particulate deposition within the bronchial tree, and the potential formation of secondary inflammatory mucus.

Furthermore, the unique physical properties of the inhaled substance may have played a role in the severity of the obstruction. It is hypothesized that the hydration of a hydrocolloid component within the gluten-free flour contributed significantly to the marked viscosity of the endobronchial material that was subsequently removed. While this specific mechanism regarding the hydrocolloid component could not be directly demonstrated through the investigative procedures, it remains a plausible contributing factor to the tenacious nature of the obstruction.

The clinical course following the intervention was successful. After the removal of the obstructing material, the child was successfully extubated and discharged from the hospital after a total hospitalization period of 20 days. This case report underscores the critical importance of meticulous history-taking in pediatric pulmonology, as seemingly innocuous environmental exposures can result in life-threatening airway obstruction that requires prompt recognition and aggressive intervention.

Acute respiratory failure in children is generally attributed to a variety of causes, including respiratory infections, asthma, trauma, foreign body aspiration, or disorders affecting the respiratory pump or central control of breathing. This case, however, highlights an unusual and underreported cause of severe respiratory compromise. The combination of massive particulate inhalation and the resulting viscous material created a unique and challenging clinical scenario for the medical team.

The findings from this case report serve as a powerful illustration of how environmental exposures, particularly those involving fine, powdery substances, can lead to severe pulmonary complications. The successful management of this patient, despite the severity of the initial presentation and the unusual nature of the obstructing material, emphasizes the necessity for clinicians to consider a broad range of etiologies when standard diagnostic pathways do not yield a clear answer.
