MINNEAPOLIS – (June 28, 2017) – From more than 8,000 proposals from 80 countries, Allina Health was chosen to present LifeCourse at the International Association of Gerontology and Geriatrics 2017 World Congress in July in San Francisco. The conference is expected to draw 6,000 professionals from around the world who are concerned with aging.
LifeCourse targets late life issues, which seriously ill people of all ages face, and are at the heart of aging boomers' experience: reduced quality of life, poor care experience, and soaring care costs. Designed and studied over a four-year time period, LifeCourse has demonstrated a savings of $959 per person per month with and an eight-to-one return on investment compared with usual care.
Allina Health researchers will present a three-part symposium:
Creating a Whole Person Intervention for Patients with Serious Illness and Their Caregivers describes the LifeCourse intervention and human-centered research approach used to create it.
Utilization, Quality of Life, and Care Experience of Patients in LifeCourse covers research results that include improved quality of life and care experience, reduced hospital use, and increased hospice length of stay.
LifeCourse Beyond Research: Learning Through Implementation and Evaluation explains how LifeCourse has been put into practice throughout the Allina Health system and at a community health center serving a diverse inner-city population.
Four additional LifeCourse papers on patient experience, quality of life, patient goals, and hospital use will also be presented. LifeCourse integrates a trained, lay navigator or community health worker called a care guide into the existing care team, freeing providers to focus more time at what they are trained best to do.
