New Verbal Social Communication Questionnaire Validated for Young Autistic Learners

Social communication lies at the heart of autism, and measuring it well in young children has long been a stubborn problem for clinicians and researchers alike. A team at The Ohio State University has now unveiled a new parent-report questionnaire, the Questionnaire for Social Communication in Young Autistic Learners, or Q-SoCiAL, that promises to change how early social communication is tracked in autistic children between the ages of two and six. Published in the Journal of Autism and Developmental Disorders, the study describes the four-phase development and psychometric validation of the tool’s verbal version, designed for children who use more than ten spontaneous words, and reports reliability figures that are exceptionally high for a measure of this kind.

The problem the researchers set out to solve is well documented. Social communication challenges, ranging from difficulty expressing wants and needs to struggles sustaining back-and-forth play, are core diagnostic features of autism, and supporting communication development is widely regarded as a fundamental priority, even a human right, by self-advocates, parents, and clinicians. Yet a 2015 review by Anagnostou and colleagues failed to identify a single measurement tool fully appropriate for use as a treatment endpoint in autism clinical trials. Existing instruments tend to suffer from limited developmental range, heavy training and administration burdens, poor sensitivity to change over short intervals, or item content better suited to diagnosis than to tracking treatment-relevant skill growth. Screening and diagnostic measures, when repurposed to track incremental progress, often confound broad developmental level with social communication and overlook meaningful gains in children with limited verbal language.

The Q-SoCiAL team approached the problem by following best-practice guidance for developing clinical outcome assessments, including standards published by the US Food and Drug Administration for patient-focused measure development. The first phase was devoted to defining the construct itself, a step the authors describe as essential because the research literature offers no single accepted definition of social communication. The diagnostic criteria for autism split social communication and social interaction into categories such as social-emotional reciprocity, nonverbal communicative behaviors, and relationships, without clearly delineating which skills belong where. Adaptive behavior frameworks such as the Vineland Scales treat communication and socialization as separate domains, which the authors argue misses the inherently social nature of communication.

To ground the definition in the perspectives of those closest to the children, the researchers ran seven focus groups with a total of 43 participants, including parents, teachers, and clinical experts. Most participants framed social communication through a skill acquisition lens and preferred strengths-based, competency-oriented measurement over deficit-focused item content. Two themes dominated the discussion: social interaction, accounting for 33 percent of all comments, and expressive communication, accounting for 30 percent, with receptive understanding contributing another 7 percent. Expert clinicians consistently distinguished social communication from structural language skill, noting that children with conditions such as Down syndrome, specific language impairment, or apraxia may show substantial language delays while retaining relatively intact social communication.

The team also cast a wide net over existing instruments, reviewing 70 questionnaire and interview measures and compiling 4,594 candidate items into a database. After screening, 4,205 items were judged potentially relevant and sorted into 619 bins across 141 sub-themes. Most existing items, however, focused on deficits or symptoms rather than competencies, so rather than adopting items directly, the researchers used this catalog to refine their construct definition and inform new item writing. Six experts in speech-language pathology, psychology, intervention, and scale development then reviewed drafts. The resulting definition centers on a child’s frequency and independent use of verbal and nonverbal behaviors during interactions, used to initiate, maintain, or respond to others in ways that are functional and serve varied communicative purposes, including requesting, sharing information, fostering social connections, and achieving reciprocal exchanges.

Crucially, the team designed the measure with neurodiversity-informed principles in mind. Items were written to capture multiple communicative modalities, including spoken words, sign language, and independent use of augmentative and alternative communication devices, all credited equally. Unconventional forms of communication, such as using another person’s hand to point or flapping hands to express happiness, are treated as positive indicators of communicative competency. The measure deliberately avoids rewarding so-called passing as non-autistic, and items about eye contact or neurotypical social performance are absent. Instead, the focus is on skills with direct functional relevance to the child’s daily life. The response scale distinguishes ability from frequency, including an “only with help” option to capture supported skill use, and prompts remind respondents that words, signs, and communication devices all count.

Phase two involved cognitive interviewing, a survey methodology technique in which respondents think aloud as they answer, exposing misunderstandings. Thirty parents and 29 teachers of autistic children completed interviews covering a pool of 103 draft items, with every item reviewed by at least three parents and three teachers. The process proved invaluable. Participants found the initial instructions too long, prompting simplification. A recurring confusion emerged over how to rate early-emerging skills, such as requesting by pointing, that a child had outgrown in favor of more complex forms like full sentences; the team responded by adding an “outgrown” response option scored at the top of the scale. Forty-six items were newly created or restructured, and 80 underwent minor revisions, yielding a final pool of 149 items across 21 facets of social communication, including joint attention, requesting, imitation, conversation, social play, and social cognition, rated over the previous month.

Phase three tested the item pool’s statistical properties. Parents of 330 autistic children, recruited largely through the Simons Foundation’s SPARK Research Match system with an additional wave to boost the number of girls, completed the full 149-item set. Item-level test-retest analysis using intraclass correlations flagged weaker items; 41 items with low stability plus other concerns were removed, along with 11 more with very low test-retest values, and redundancy screening identified 16 item pairs correlating above 0.80. The surviving 46 items then underwent exploratory factor analysis using the weighted least squares mean and variance adjusted estimator, appropriate for ordinal response data. The first eigenvalue explained roughly 60 percent of total variance, and although parallel analysis suggested up to three factors, the one-factor solution showed the best fit and clinical interpretability, with a comparative fit index of 0.948, a Tucker-Lewis index of 0.945, a root mean square error of approximation of 0.079, and a standardized root mean square residual of 0.083. In practical terms, a simple sum score captures the construct, a convenience for busy clinics.

The reliability figures are striking. Internal consistency reached a Cronbach’s alpha of 0.98. Test-retest reliability over an average interval of 12 days produced an intraclass correlation of 0.96, indicating that scores are highly stable across a period in which little real change would be expected. Cross-rater agreement was similarly strong, at 0.83 between two parents and 0.86 between a parent and an educator such as a teacher, therapist, or childcare provider, suggesting that the behaviors measured are observable and consistent across settings and raters. Scores spanned nearly the full theoretical range of 0 to 138 in the autistic sample, with a mean of 80.0, and correlated strongly with parent-rated language level, r = 0.792, and modestly with chronological age, r = 0.262, while showing no significant association with child sex, parent age, or family income.

Phase four examined external validity. In a comparison of 330 autistic children with 401 neurotypical children, an analysis of covariance controlling for age showed a large main effect of group, indicating that autistic children scored lower overall even after accounting for language level and age, with all pairwise contrasts significant after Bonferroni correction. A small in-person clinic subsample of 19 autistic children provided convergent evidence: Q-SoCiAL scores correlated moderately with nonverbal and verbal IQ on the Stanford-Binet, moderately with the Vineland Communication and Socialization domains, and strongly and negatively with Childhood Autism Rating Scale scores, while showing small, non-significant correlations with Vineland Daily Living and Motor domains and with the Behavioral Inflexibility Scale, a pattern consistent with divergence from unrelated constructs.

The authors are careful to note the measure’s limitations. Sensitivity to change over time and following intervention has not yet been directly tested, and convergent validity rests on a small sample. A companion version for preverbal children and those using ten or fewer words, the Q-SoCiAL-Early Communication Version, is under development, and measurement invariance across gender, language backgrounds, and racial and ethnic groups requires further study. Even so, the tool fills a conspicuous gap. It can be completed by an untrained parent, teacher, or clinician in under ten minutes, and it offers a fine-grained, strengths-based portrait of skills that early interventions, from naturalistic developmental behavioral approaches to speech therapy, actually target. For a field long hampered by measures that cannot detect the progress that matters most to families, the Q-SoCiAL may prove to be exactly the sensitive, practical instrument that clinical trials and community services have been waiting for.
