Hypertension Patients More Willing to Take Medication Than Change Lifestyle

Philadelphia—A diagnosis of high blood pressure may make people more willing to take prescribed medication while making them less inclined to adopt the lifestyle changes doctors routinely recommend, according to a new study published in the American Heart Association journal Hypertension. The research suggests that medication adherence and healthy behavior may be guided by different psychological processes, challenging the assumption that patients who accept one part of hypertension treatment will automatically embrace the others.

The study, led by Yotam Ophir of the University at Buffalo, State University of New York, analyzed responses from a nationally representative sample of 1,398 U.S. adults. Researchers examined participants’ intentions to follow medical advice in two broad areas: taking blood-pressure medication and making lifestyle changes, including exercising regularly, maintaining a healthy weight, improving diet, and reducing salt consumption. The findings were developed using the Health Belief Model, a longstanding framework in public health research that explains health behavior through people’s perceptions of risk, disease severity, treatment benefits, and obstacles to action.

The results revealed a striking difference between the two types of treatment intentions. Traditional health beliefs predicted intentions to take medication two to three times more effectively than they predicted intentions to adopt healthier habits. Participants who believed hypertension posed a serious threat, who felt personally vulnerable to its consequences, or who thought treatment would be beneficial were more likely to say they intended to take medication. Those same beliefs, however, were substantially weaker predictors of whether people planned to change their diet, exercise patterns, weight-related behaviors, or salt intake.

A hypertension diagnosis itself was associated with greater intentions to follow recommendations about medication. This pattern was particularly evident among people who expressed concern about having hypertension or who believed that medication could effectively control the condition. The diagnosis may make drug treatment feel concrete and medically authoritative: a prescription offers a defined action, a measurable dose, and a direct connection to clinical care. Lifestyle change, by contrast, can involve numerous daily decisions and may appear more difficult to begin or sustain.

Even after researchers accounted for participants’ willingness to take medication, people diagnosed with high blood pressure reported weaker intentions to follow recommendations for lifestyle improvement. The finding does not mean that every person with hypertension avoids exercise or healthy eating, nor does it demonstrate that a diagnosis directly causes unhealthy behavior. Instead, because the research relied on survey responses, it identifies an association between diagnosis status and stated intentions. Whether those intentions translate into long-term behavior would require follow-up studies using clinical measurements, activity records, dietary assessments, or other behavioral data.

The researchers propose that lifestyle changes may be influenced by factors that extend beyond the beliefs traditionally emphasized by the Health Belief Model. Exercise, weight management, nutrition, and salt reduction require repeated effort, access to healthy food and safe spaces, time, money, social support, and confidence in one’s ability to maintain new routines. Medication, while it can also present barriers, is often framed as a single clinical intervention. This difference may help explain why communicating the dangers of hypertension can increase support for medication without producing an equivalent commitment to broader behavioral change.

Communication between patients and healthcare providers also emerged as an important factor. In most analyses, respondents who reported greater barriers to discussing health information with physicians were less likely to say they intended to follow recommendations, whether those recommendations involved medication or lifestyle. Communication barriers can include difficulty asking questions, uncertainty about medical terminology, limited time during appointments, discomfort discussing personal habits, or a belief that a clinician will not understand a patient’s circumstances. When these obstacles are present, patients may leave consultations without a clear or practical plan.

High blood pressure affects more than 100 million adults in the United States and approximately one billion people worldwide. Often called a “silent killer,” hypertension frequently produces no noticeable symptoms while gradually increasing strain on blood vessels and vital organs. Persistently elevated pressure can damage the heart, brain, kidneys, and eyes and substantially increase the risk of heart attack, stroke, heart failure, and other cardiovascular complications. Clinical guidelines therefore generally recommend a combination of medication, when appropriate, and lifestyle measures such as a nutritious diet, regular physical activity, healthy weight management, and reduced sodium intake.

The study’s authors say the findings point toward more tailored approaches to hypertension counseling. Rather than assuming that a diagnosis, a warning about cardiovascular risk, or a prescription will motivate every form of treatment, clinicians may need to address the practical and psychological demands of each behavior separately. Future interventions could combine clear risk communication with specific, achievable goals, frequent follow-up, collaborative problem-solving, and support adapted to a patient’s daily environment. “Medication and lifestyle changes are both essential components for treatment of hypertension,” Ophir said, “but our findings suggest they may be driven by different psychological processes.” The research letter, titled “Hypertension Treatment Intentions: A Health Belief Model Analysis,” was published July 2, 2026, with funding from the Annenberg Public Policy Center’s Annenberg Health and Risk Communication Institute and the National Institutes of Health.
