Primary Care Doctors Compare Tradeoffs Between Salaried Employment and Private Practice

A new qualitative study published in The Annals of Family Medicine spotlights how day-to-day practice differs between U.S. primary care physicians working for health systems and those in independent settings. Drawing on in-depth interviews with 30 physicians, the researchers identified five recurring themes that capture the practical trade-offs behind the growing shift toward employed models of care.

Across the interviews, health system physicians reported reduced autonomy over scheduling and clinic operations. In exchange, they described stronger financial and legal safeguards. The study suggests that these institutional protections come bundled with more standardized workflows, which can limit individual control over how care is delivered.

A second theme concerned clinical infrastructure. Physicians employed by health systems often had embedded support teams and easier pathways to specialists, reflecting the operational capacity of larger organizations. Independent physicians, by contrast, described managing care with comparatively limited staffing, relying more heavily on personal networks and manual coordination.

The researchers also highlighted how information systems reshape collaboration. Shared electronic health records and integrated referral networks in health systems enabled smoother care coordination and faster specialty access. Independent physicians were more likely to depend on manual outreach, phone calls, and professional relationships to bridge gaps in communication.

Cultural dynamics emerged as another dividing line. Health system practices, participants said, frequently operate within performance-driven environments that emphasize productivity metrics. This emphasis can alter clinical pacing and decision-making priorities, even when physicians maintain high standards of patient care.

Independents, however, described a different professional focus: protecting time and continuity with patients. Many reported that their practice model supports longer-term relationships, allowing them to monitor health trajectories with fewer organizational constraints.

Communication structures were also central to the patient experience. Centralized routing in health systems, physicians reported, can delay access to the right staff member. In independent practices, patients more often reach familiar personnel directly, potentially improving responsiveness during urgent or evolving concerns.

Finally, the implications extend beyond individual workplaces. With 55% of U.S. physicians now employed by hospital and health systems—nearly a 50% increase since 2012—the findings frame an urgent question for health policy: how will expanding employment affect clinical autonomy, coordination efficiency, and patient access?
