Breast Milk Odor Exposure Influences Early Oral Feeding in Preterm Infants

Breast milk is known for its nutritional power, but new research suggests it may also carry subtle behavioral cues that shape how preterm infants begin feeding. Investigators have reported that odor exposure to breast milk can influence feeding performance during the first attempt at oral feeding—an especially critical period for babies who are not yet fully mature.

In the study, preterm infants were exposed to breast milk odor following non-nutritive sucking, a common clinical practice that helps prepare infants for oral feeding. Non-nutritive sucking provides sensory and motor rehearsal without calories, potentially priming infants’ oral skills before true feeding begins.

The researchers focused on “first oral feeding performance,” evaluating how effectively infants responded once oral feeding was initiated after the odor manipulation. This approach targets a real-world transition point, where even small sensory differences—like scent—may alter readiness and coordination.

Odor cues are particularly relevant because early sensory systems are actively developing. Chemical signals in milk can interact with the infant’s olfactory pathways, which are already functioning before full feeding competence is established. By leveraging that sensory input, the study tests whether scent acts as a form of non-caloric guidance.

Analytically, the work builds on the idea that conditioned responses can form even without nutritional intake. If breast milk odor reliably increases engagement with feeding behaviors, clinicians could potentially standardize sensory preparation to improve outcomes during early feeding trials.

The findings, published in J Perinatology, add to growing evidence that feeding readiness is not purely mechanical. Instead, it may reflect a coordinated system involving sensory perception, autonomic regulation, and oral-motor control—especially during the earliest feeding exposures.

Taken together, the results may influence how NICUs design feeding protocols. If validated across larger cohorts, incorporating odor-based preparation could be a low-cost, non-invasive strategy to support preterm infants at a vulnerable milestone.

For parents and clinicians watching the first oral feeding closely, the implication is striking: breast milk may “announce itself” through scent, helping infants respond with better initial performance when it matters most.
