Randomized Trial Tests Advance Care Planning and Coordination for Parkinsonism Patients

A new randomized controlled trial is putting advance care planning (ACP) and care coordination under a scientific spotlight for people living with parkinsonism. The study, published in 2026, evaluates whether a structured approach can strengthen end-of-life readiness, improve alignment of care with patient goals, and reduce the fragmentation that often follows progressive neurologic disease.

The researchers, led by Dijkstra and colleagues, enrolled participants with parkinsonism and randomly assigned them to receive an intervention centered on ACP support paired with proactive coordination. Instead of treating planning as a one-time conversation, the program emphasized guided discussions, documentation of preferences, and follow-through between care settings.

What makes the trial technically notable is the integration of ACP with coordination mechanisms designed to translate expressed preferences into clinical action. This includes building continuity across professionals and clarifying roles so that care decisions are less likely to drift from the individual’s documented values as symptoms evolve.

Investigators also focused on outcomes that reflect real-world complexity, such as whether patients experience improvements in preparedness and whether the care trajectory becomes more coherent for clinicians and families. The trial framework allows the team to distinguish signal from noise by comparing outcomes directly against a control group without the tailored ACP-plus-coordination package.

Because parkinsonism can involve fluctuating disability, comorbidities, and caregiver burden, timing is critical. The intervention was structured to occur early enough to be meaningful, yet responsive to changes over the course of illness—an approach intended to increase the odds that preferences remain relevant rather than becoming outdated.

Trial results reported in npj Parkinson’s Disease highlight the potential for ACP to function beyond documentation. When paired with coordination, ACP may act as an operational bridge between patient wishes and the logistics of healthcare delivery, helping teams anticipate needs and coordinate timely services.

In an era where clinician time and system capacity are constrained, scalable care models are increasingly valuable. If the findings hold across broader settings, they could inform how neurology clinics and allied services design future care pathways for progressive disease.

For readers, the takeaway is straightforward: advance care planning may be more effective when it is not isolated from the healthcare network around the patient. This study offers a blueprint for turning conversations into coordinated care processes—and doing it with the rigor of a randomized trial.
