Phase-Sensitive Framework Integrates Four Methodologies for Health System Change

Researchers have proposed a phase-sensitive framework to integrate implementation science, quality improvement, Biodesign, and comparative effectiveness research across health system change trajectories, according to a perspective published in Frontiers in Health Services. Authored by John Tshon Yit Soong and colleagues across Singapore institutions including the National University Hospital and Duke-NUS Medical School, the perspective addresses how aligning these distinct disciplines can support care transformation from unmet clinical needs to sustainable delivery.

According to the authors, modern health systems increasingly rely on implementation science, quality improvement, Biodesign, and comparative effectiveness research to develop, evaluate, implement, and sustain improvements in care. Although each field offers distinct methodological strengths, they are frequently applied in isolation. This separate application creates discontinuities across the overall change trajectory, leaving healthcare leaders with limited practical guidance on how to combine their complementary contributions.

To overcome these disciplinary silos, the proposed framework organizes methodological contributions according to the functional tasks of health system change rather than disciplinary ownership. Rather than assigning individual traditions to rigid stages, the model demonstrates how approaches from all four traditions can be selectively combined to satisfy the methodological requirements of each operational phase.

The framework categorizes these functional contributions across four recurring phases: problem framing and system understanding; solution design and selection; evaluation and adaptation; and embedding, sustaining, and scaling. To illustrate how these methods interact across every stage, the authors present an application focused on remote monitoring after hospital discharge for heart failure.

The authors note that the framework provides a conceptual structure for determining when and how to combine complementary methods while preserving their distinct purposes and analytical strengths. However, they emphasize that the practical utility and impact of the framework require empirical evaluation across diverse health system contexts to confirm whether phase-sensitive integration can deliver effective, equitable, and sustainable change.
