Study Links miR-1224-5p and SND1 to Systemic Inflammation Following Hip Arthroplasty

Serum miR-1224-5p levels are decreased in patients who develop systemic inflammatory response syndrome (SIRS) following total hip arthroplasty (THA) compared with patients in the non-SIRS group, according to research published in BMC Immunology. Total hip arthroplasty is recognized as the optimal treatment for femoral neck fracture (FNF), but the inflammatory response triggered by surgical trauma remains a significant concern. To investigate the functions and mechanisms of miR-1224-5p in post-THA SIRS, researchers analyzed a clinical study cohort comprising 300 FNF patients undergoing THA, of whom 128 developed SIRS after the surgical procedure.

The investigators quantified miR-1224-5p and SND1 levels in the patient cohort using reverse transcription-quantitative polymerase chain reaction (RT-qPCR). In parallel laboratory investigations, the researchers examined cellular responses in THP-1 macrophages exposed to lipopolysaccharide (LPS). LPS treatment impaired THP-1 macrophage viability, activated inflammatory responses, and activated the NF-κB signaling pathway. Cell viability was assessed via CCK-8 assay, inflammatory markers were detected using ELISA kits, and NF-κB pathway activation was evaluated by Western blotting.

Modulation of miR-1224-5p expression demonstrated distinct effects on macrophage behavior. Treatment with a miR-1224-5p mimic improved LPS-induced THP-1 cell function, whereas a miR-1224-5p inhibitor further suppressed the biological functions of the cells. Mechanistically, binding interactions between miR-1224-5p and SND1 were validated through luciferase activity and RIP assays, identifying SND1 as a direct downstream target of miR-1224-5p. Subsequent experiments demonstrated that up-regulation of SND1 partially reversed the protective properties of miR-1224-5p on THP-1 cells.

The study concluded that miR-1224-5p may be associated with the post-THA inflammatory response, potentially involving SND1 and accompanied by alterations in the NF-κB pathway. The authors cautioned, however, that a strict linear axis or target dependence cannot be concluded from the current data. While miR-1224-5p is expected to serve as a promising biomarker and therapeutic candidate for further investigation, the researchers highlighted that cell-based assays and observational patient comparisons represent early mechanistic findings rather than established clinical applications.
